Title | Failure to Perform a 50.59 Screening for Changing the Frequency of Exercising the Turbine Bypass Valves |
Description | The inspectors identified a Severity Level IV NCV of 10 CFR 50.59 4(d)(1), Changes, Tests, and Experiments, and an associated Green finding for the licensees failure to perform a written evaluation which provided the bases for determining that changing the turbine bypass valve surveillance testing frequency from every 31 days, as specified in the Updated Safety Analysis Report, to once a year did not require a license amendment. The licensee has entered this issue into their corrective action program as AR 02720163. The licensee is currently evaluating the issue in accordance with their procedure for changes to the facility. The inspectors determined that the licensees failure to perform a written evaluation to provide the basis for the determination that a change to the facility, a change to a procedure, or a change to a test or experiment did not require a license amendment was a performance deficiency. The performance deficiency was more than minor in accordance with IMC 0612, "Power Reactor Inspection Reports," Appendix B, "Issue Screening," dated September 7, 2012, because, it was associated with the procedure quality attribute of the Mitigating Systems cornerstone and adversely affected the cornerstone objective of ensuring the availability, reliability, and capability of systems that respond to initiating events to prevent undesirable consequences. Using IMC 0609, Attachment 4, Initial Characterization of Findings, and Appendix A, The Significance Determination Process for Findings At-Power, issued June 19, 2012, the finding was screened against the Mitigating Systems cornerstone and determined to be of very low safety significance because all of the associated questions in IMC 0609, Appendix A, were answered no. Violations of 10 CFR 50.59 are dispositioned using the traditional enforcement process because they are considered to be violations that potentially impede or impact the regulatory process. The inspectors reviewed Section 6.1.d.2 of the NRC Enforcement Policy and determined this violation was Severity Level IV because the resulting changes were evaluated by the SDP as having very low safety significance. The inspectors determined this finding affected the cross-cutting area of human performance, in the aspect of consistent process, where individuals use a consistent, systematic approach to make decisions. The licensee made a decision to proceed with implementation of a change to the turbine bypass valve surveillance testing frequency after a plant oversight committee review in lieu of following their consistent, systematic process for evaluating changes to the USAR.
