Failure to Incorporate Design Information Into System Test Procedures

Description

The inspectors identified a non-cited violation of 10 CFR Part 50, Appendix B, Criterion XI, Test Controls, for the licensees failure to ensure that the stations in-service testing program for main steam isolation valves (MSIVs) incorporated the requirements and acceptance limits contained in applicable design documents. Specifically, the licensees in-service procedures did not direct testing of the valves be performed at the minimum required pressure and this resulted in the licensees failure to identify two degraded MSIVs during in-service testing. The licensee entered this issue into the corrective action program as Condition Report CR-2018-003229.
