| Description | The following violation of very low safety significance (Green) was identified by the licensee and is a violation of NRC requirements, which meets the criteria of Section VI of the NRC Enforcement Policy, NUREG-1600, for being dispositioned as an NCV. Technical Specification 5.4, Procedures, required the implementation of the applicable procedures recommended in Regulatory Guide 1.33, Quality Assurance Program Requirements (Operation), Revision 2, dated February 1978. Regulatory Guide 1.33,Appendix A, Part 9a, stated, Maintenance that can affect the performance of safety-related equipment should be properly preplanned and performed in accordance with written procedures, documented instructions, or drawings appropriate to the circumstances. Contrary to this requirement, on March 11, 2009, the licensee failed to adhere to procedures and performed work on the \'B\' RHR system when the procedures specified work on the \'A\' RHR system. The \'B\' RHR system was considered available as a backup system for spent fuel pool cooling at the time of the event. The finding was determined to be of very low safety significance because the reactor vessel was defueled and the finding did not meet IMC 0609 Appendix G criteria for quantitative assessment.
