Two Examples of Inadequate Operability Evaluations

Description

This finding is more than minor because the failure to perform adequate operability evaluations, if left uncorrected, could become a more significant safety concern. Using the Manual Chapter 0609, Significance Determination Process, Phase 1 Worksheet, this finding was of very low safety significance since it did not result in a loss of operability, nor did it screen as potentially risk significant due to a seismic, flooding, or severe weather-initiating event. The cause of this finding has a crosscutting aspect in the area of human performance associated with decision making because licensee personnel failed to use conservative assumptions and did not verify the validity of the underlying assumptions used in making safety-significant decisions H.1(b).
