Severity Level IV non-cited violation of 10 CFR Part 50 Appendix B Criteria VII and XV, for the licensees failure to ensure materials intended for installation in safety-related applications conformed to procurement requirements or, if they did not, were adequately controlled and evaluated. The failure to establish a program to evaluate and control nonconforming materials in accordance with the procurement requirements of 10 CFR 21 was a performance deficiency. This performance was more than minor because if left uncorrected it had the potential to become a more significant safety concern. Using
Inspection Manual Chapter 0609 Appendix A, dated June 19, 2012, the team determined that this finding was of very low safety significance (Green) because it was a def iciency affecting the design or qualification 3 of a structure, system, or component, and operability was maintained. The finding has a conservative bias cross-cutting aspect in the human performance cross-cutting area because licensee personnel improperly rationalized the adequacy of the nonconforming components to perform their safety-related functions
