EVENT DESCRIPT10N' On May 26, 1982, 6 floor drain and equipment drain sumps were inspected.

The inspection consisted of determining if the subfloor drain' piping had

~

a closure seal at the point it penetrated the sump liner.

No sealing welds were found on the lines that penetrated the sump liners.

V.

PROBABLE CONSEQUENCES OF THE OCCURRENCE Af ter a long period of time there is a low probability that radioactive effluents could have been released to the environment through the process

, of leaching through the concrete if the seals were not in place.

Since the required seals were completed before initial criticality and no-contamination has ever been present in the sunps, no radioactive material has been or will be released via this path.

VI.

CAUSE

Drawings A-529 and A-598 Indicated that the liners should have been sealed at the penetration but no record of installing the seals could be located.
