| Description | Technical Specification 3.3.7, Diesel Generator Undervoltage Start, requires that four channels of loss of voltage function and four channels of degraded voltage function auto-initiation instrumentation per diesel generator be operable. Contrary to the above, from January 28 to April 8, 2011, Unit 3 was operated with one or two train B 3A0615 loss of voltage signal channels inoperable for a period of time longer than allowed by Technical Specification 3.3.7. This issue was entered into the licensees corrective action program as Nuclear Notifications NNs 201410580 and 201412748. |
