| Description | 10 CFR 50 Appendix B, Criterion V, Instructions, Procedures, and Drawings, states, in part, that activities affecting quality shall be prescribed by documented instructions, procedures, or drawings of a type appropriate to the circumstances and shall be accomplished in accordance with those instructions, procedures, or drawings. Contrary to the above, on March 7, 2013, Dominion failed to maintain a HELB door closed during the TDAFW pump surveillance and rendered both trains of AFW inoperable for approximately 30 minutes. The inspectors determined that the finding was of very low safety significance (Green) in accordance with IMC 0609, Appendix A, The Significance Determination Process for Findings at Power. Dominion entered the issue into their CAP (CR507412). |
