Title | Inadequate Corrective Action for 2010 Degraded Voltage Issues |
Description | The team identified a non-cited violation of 10 CFR 50, Appendix B, Criterion XVI, Corrective Action, for the licensees failure to promptly identify and correct deficiencies in electrical calculations for the safety-related AC electrical distribution system identified during the 2010 CDBI. Specifically, the licensees failure to identify that safety-related motor operated valve (MOVs) needed to be evaluated for new lower calculated available voltage (degraded voltage) to ensure their operability was a performance deficiency. The licensee initiated PER 753504 and performed a prompt determination of operability (PDO). The team concluded that the evaluations and compensatory measures described in the PDO provided reasonable expectation of operability. The performance deficiency was determined to be more than minor because it affected the Design Control attribute of the Mitigating Systems cornerstone, and adversely affected the cornerstone objective of ensuring the availability, reliability, and capability of systems that respond to initiating events to prevent undesirable consequences. Specifically, failure to identify and evaluate that safety-related MOVs could be affected by degraded voltage conditions did not ensure the availability, reliability, and capability of the MOVs to respond to initiating events. The team used Inspection Manual Chapter 0609, Significance Determination Process, Attachment 4, Initial Characterization of Findings, and Appendix A, The Significance Determination Process for Findings At-Power, and determined that the finding was of very low safety significance (Green) because the finding was not a design deficiency resulting in the loss of functionality or operability. The team determined that this finding represented present licensee performance and directly involved the cross-cutting area of Problem Identification and Resolution, component of Corrective Action Program because the licensee failed to identify that safety-related MOVs needed to be evaluated for new lower calculated available voltage (degraded voltage) to ensure their operability. |
