Description | Technical Specification 3.0.4.a required that when an LCO is not met, entry into a Mode or other specified condition in the applicability shall only be made when the associated actions to be entered permit continued operation in the Mode to be entered for an unlimited period of time. Contrary to this, from initial power operations until August 2008, the licensee had entered Modes 2 and 1 with one main feedwater pump trip channel for the AFW automatic start function inoperable when the associated actions of TS 3.3.2 did not permit operation for an unlimited period of time in this condition. The licensee entered this issue into their corrective action program as PER 150982. The licensee subsequently requested and received a license amendment that revised the applicability of TS 3.3.2. This finding is of very low safety significance because it did not represent an actual loss of safety function since other initiation signals remained available to automatically start the AFW pumps if needed |
