LER ATTACHMENT - RO #2-82-124 Facility: BSEP Unit No. 2 Event Date: November 21, 1982 On November 21, 1982, at 1800 hours0.0208 days <br />0.5 hours <br />0.00298 weeks <br />6.849e-4 months <br />, an LCO was initiated on diesel generator No. 3 to permit the performance of monthly periodic testing on the diesel.

Due to logic problems identified in LER 2-82-123, the periodic testing on diesel generator No. 3 necessitated quick start testing of the remaining diesels (Nos. 1, 2, and 4) within two hours. Due to a misinterpretation of the plant standing instructions regarding the performance of required diesel generator quick starting, the responsible Shift Foreman failed to perform the required quick start testing. Diesel generator No. 3 was out of service for two hours and eight minutes. Discovery of this event was not made until November 22, 1982, when discussion between the Shift Foreman and the plant Regulatory Compliance support group determined that technical specifications had been exceeded. Satisfactory quick starts were then performed on diesel generator Nos. 1, 2, and 4 on November 22, 1982, at 1320 hours0.0153 days <br />0.367 hours <br />0.00218 weeks <br />5.0226e-4 months <br />.

As a result of this event, the plant standing instructions have been revised to provide better detailed instruction on the required diesel generator quick starting requirements.

It is felt this will prevent future similar events.

All plant licensed Operations personnel will review this report to ensure their familiarity with it and the importance of making proper interpretation of plant instructions.
