Failure to Verify Safety-Related 4160 V Breaker Operability Following Maintenance Activities

Description

The inspectors reviewed a self-revealed Green finding and associated non-cited violation of Arkansas Nuclear One, Unit 1, Technical Specification 5.4.1.a, for the licensees failure to properly preplan maintenance that can affect the performance of safety-related equipment. Specifically, the licensee failed to perform post-maintenance testing to demonstrate component operability for the train A safety-related 4160 V switchgear A-303 breaker that provides power to the swing service water pump B (P-4B) after the breaker was racked in. The breaker subsequently failed to close when attempting to start the pump.
