This violation of very low safety significance was identified by the licensee and has been entered into the licensees corrective action program and is being treated as a
NCV, consistent with Section 2.3.2 of the
Enforcement Policy. Violation:
10 CFR Part 50, Appendix B, Criterion V, Instructions, Procedures, and Drawings, requires, in part, that activities affecting quality shall be prescribed by documented instructions appropriate to the circumstances and shall be accomplished in accordance with the instructions. Contrary to the above, from January 1994 to June 2017, Entergy modified site surveillance procedure 8.M.3-18,
Standby Gas Treatment System Exhaust Fan Logic Test and Instrument Calibration, without prescribing adequate documented instructions for the condition caused by the testing. Specifically, Entergy failed to identify that the procedurally prescribed lineup of the
standby gas treatment system resulted in
secondary containment being
inoperable due to the large opening introduced into the system. Significance/Severity: The inspectors evaluated this finding using
Inspection Manual Chapter 0609.04, Initial Characterization of Findings, and
Inspection Manual Chapter 0609, Appendix A, Exhibit 3,
Barrier Integrity Screening Questions. The inspectors determined that the finding was of very low safety significance. Corrective Action Reference:
