| Description | A self-revealing, NCV of very low safety significance (Green) of Technical Specification (TS) 6.4, Procedures, was identified when operators inadvertently drained water from the reactor pressure vessel (RPV) during integrated emergency core cooling system (ECCS) testing. Specifically, Entergy failed to establish the initial plant conditions necessary to perform integrated ECCS testing without causing an inadvertent drain down of the vessel through the main steam lines, the RCIC turbine, and into the torus. Entergy restored the RPV inventory, initiated a CR to perform a root cause evaluation of the issue, and assigned a corrective action to revise the procedure in order to preclude recurrence in future outages. The inspectors determined that the finding was more than minor because it was associated with the Procedure Quality attribute of the Initiating Events Cornerstone, and affected the cornerstone objective to limit the likelihood of those events that upset plant stability and challenge critical safety functions during shutdown as well as power operations. Specifically, the implementation of the inadequate procedure guidance resulted in an unexpected loss of RPV water inventory of approximately 2100 gallons. The inspectors determined that this finding had a cross-cutting aspect in the Human Performance cross-cutting area, Resources component, because the test procedure was inadequate. Specifically, the procedure did not provide adequate directions for establishing plant conditions during a test that had the capability of draining RCS inventory H.2(c). |
