Title | Licensee-Identified Violation |
Description | TS 3.9.4.c requires in part that during fuel movement, all containment penetrations shall be closed or capable of being closed by an automatic valve. In addition, TS 3.9.4.c allows exceptions to this requirement for penetrations that traverse to the auxiliary building secondary containment enclosure (ABSCE) where these penetrations may be open under administrative controls during fuel movement. Contrary to the above, on several instances between 2000 and 2014, the licensee opened penetrations (between containment and ABSCE) during fuel movement without adequate administrative controls in place. The finding was considered more than minor because it was associated with the Barrier Integrity cornerstone and affected the cornerstones ability to preserve the containment boundary. The inspectors determined that, although the finding involved a violation of the containment control, TS 3.9.4, the finding did not: 1) involve a loss of reactor coolant system (RCS) inventory; 2) degrade ability to terminate a leak path or add RCS inventory as needed; or 3) degrade the ability to recover RHR once it was lost. Therefore, according Appendix G, the finding did not require a quantitative (phase 2 or 3) analysis. Findings in the shut-down condition that do not require a quantitative analysis are considered to be of very low safety significance (Green). This issue was entered into the CAP as PER 886970. Note that this issue is also discussed under Section 4OA3 of this report as it involved a LER. |
