HEAT TRACING "FIULURE With the Unit at full p::,weI' and dur:ing a check of low heat tracing alanns, circuit 2A Panel 11, #2 BIT inlet, was found to have low amps.

This is contrary to T.S

  • 3.3.A.4. an:l is re:i;:ortable per T.S. 6.6.2.b.(2).

PIDBABLE mNSE~S:

The heat tape is intended to rna:inta:in the J:oric acid at a tanperatnre such that soluability, and hence flow, is ma:inta:ined.

'!he redundant channel of heat tape was verified operat:ing. Therefore the health and safety of the p.lblic were not affected.

CAUSE

The. heat-trac:ing tape was darraged by individuals \\\\Crking pipe supp::,rt :rrodifications in the nearby area.

  • J1.'.MEDIATE CORRECTIVE ACI'ION:

The irrrnediate cor.rective action was to verify operation of the redundant heat tracing circuit.

SUBSEQJENT -CORRECTIVE ACTION:

SUbsequent Cor.rective action was to repl~e the*.cllanage:li heat tape :and verify it operable.

6.

ACTIONS TAKEN TO PREVENT RIDJRRENCE:

Administrative and organizational changes have been initiated to prevent such occurrences.

Refer to LER 80-030/03L-O, 11-4-80, Unit No.2.
