6.9.1.12.1 Date of Occurrence: 3/14/80 Time of Occurrence: 1700 EST Unit 1 Identification and Description of Occurrence:

Letter from Westinghouse (NS-TMA-2214) notified TVA of the potential for guide tube support pin cracking.

Conditions Prior to occurrence:

Unit 1 shutdown in mode 6.

Action specified in the Technical Specification Surveillance Requirements met due to inoperable equipment. Describe.

N/A Apparent Cause of Occurrence:

Improper heat treatment of pins.

Analysis of Occurrence:

If the pins had failed during operation and an accident had occured which required UHI initiation it would have been possible for guide tube misalignment and subsequent RCCA failure.

Corrective Action

Replacing potentially defective guide pins.
