i Identification and Description of Occurrence During a recent pipe hanger inspection, it was discovered that a pipe hanger from a previous design revisien had not been removed from an auxiliary feedwater system pipe. This constituted a hanger configuration which had not been seismically analyzed.

Conditions Prior to Occurrence Mode 5 and filling and venting RCS.

Action Specified in the Technical Specification Surveillance Requirements Met Due to Inoperable Eauipment N/A Apparent Cause of Occurrence Personnel error. Pipe hanger should have been removed.

Analysis of Occurrence N/A

Corrective Action

The pipe hanger installed from the previous design revision was removed; therefore, the system should operate with no adverse effects as designed.
