ITEM:
9-12, 1978, it Based on the results of the NRC inspection conducted on May appears that certain of your activities were not conducted in full compliance These items have been categorized with NRC requirements as indicated below.

as described in our correspondence to you dated December (2) Each entrance or access point 10 CFR 20.203(c) states in part:
(i) Equipped with a control to a high radiation area shall be:
device which shall cause the level of radiation to be reduced...;
or (ii) Equipped with a control device which shall energize a conspicuous visible or audible alarm signal...; or maintained locked except during period when access to the area is required, with positive control over each individual entry.... (4) In the case of a high radiation area established for a period of 30 days or less, direct surveillance to prevent unauthorised entry may be substituted for the control required by subparagraph (2) of this paragraph."
Contrary to the above, a high radiation area surrounding a de-watered resin cask, located outside the Unit 2 auxiliary building was not under direct surveillance from 11:00 a.m. on May 9, 1978 to 12:00 p.m. on May 11, 1978 nor was the area locked or alarmed.
RESPONSE
An EP technician was promptly assigned to provide direct eurveillance of the established high radiation area until security personnel atrived.
Failure to follow previously established HP procedures resulted in disciplinary action for the technician involved in this violation.
Written instructions have been distributed which reiterate the required actions h
to be taken by Health Physics and security personnel in the control of hig radiation areas.

In accordance with Section 2.790 of the NRC's " Rules of Practice,"
Part 2, Title 10, Code of Federal Regulations, a copy of this letter and the enclosed inspection report will be placed in the NRC's Public Document Room. If this report contains any information that you (or your contractor) believe to be proprietary, it is necessary that you make a written application within 20 days to this office to withhold such information from public disclosure.
Any such application must include a full statement of the reasons on the basis of which it is claimed that the information is proprietary, and should be prepared so that proprietary information identified in the application is contained in a separate part of the document.
If we do not hear from you in this regard within the specified period, the report will be placed in the Public Document Room.
Should you have any questions concerning this letter, we will be glad to discuss them with you.
