Conditions Prior to Occurrence:

Unit 1 in Mode 6 following initial core leading.

Action specified in the Technical Specification Surveillance Requirements met due to inoperable eculpment.

Incorrect valve operator weight used in seismic analysis.

Analysis of Occurrence:
