Description of Event

On October 25, 1980 while operating in Mode I at-100% power, the temperature indicator (TI-2412B) associated with the overpower AT setpoint for Loop A failed low.

The applicable Technical Specification is 3.3.1.1,

- and the event is reportable pursuant to T.S. 6.9.1.9.c.

Probable Consequences of Occurrence Since the affected channel was placed in the trip condition within one hour, as required by T.S. 3.3.1.1, the health and safety of the public were not affected.

1

Cause of Event

The temperature setpoint failed low when leads from a recorder were connected to unisolated points of the circuit. The corrective action was to change the procedure of the load swing test so that the recorder i

leads are connected to isolated points of the circuit.

Immediate Corrective Action

The immediate action was to place the overpower AT channel in the trip condition. The channel was checked to verify it was not affected by the recorder leads.

Scheduled Corrective Action

~There are no scheduled corrective actions.

Actions Taken to Prevent Recurrence A change was made in the procedure of the load swing test such that leads from the. recorder are connected ~at isolated points of the appropriate circuits.

Generic Implications 4

There are no generic implications.
