These events constitute a reportable occurrence as defined in the Technical Speci fications, paragraph 6.9.2. A.6.

However, this fact was not recognized as such until August 6, 1980.

Conditions Prior to Occurrence Startup testing following a reueling/ maintenance outage was in progress. The major parameters at the time of occurrence were:

Feedwater, 1.2 x 106 lbs/hr Recirculation, 5 3 x 104 gpm 0

Stack Gas: 0.7 x 10 uct/sec I

l Description of Occurrence On July 16, 1980 at approximately 4:30 p.m.,

an operator performing a routine tour of thi Reactor Building discovered that both doors from the torus area to tae con-tainment spray pump areas (northeast and southeast corners of the Reactor Building

The operator immediately closed the doors and dogged them shut and reported the incident to the Group Shift Supervisor.

Reportable Occurrence No. 50-219/80-32/IT August 14, 1980 Page 2 Accarent Cause of Occurrence Although signs are posted on each door indicating that they must be closed and dogged at all times except for passage, there is an apparent lack of administrative controls to ensure that this is done. Personnel workins in the areas inadvertently left the doors open and failed to recognize the fact that they were in violation of the Technical Specifications.

Analysis of Occurrence The containment spray cooling system consists of two independent :coling loops, each capable of removing heat from the primary containment following a loss of coolant accident. Since bcth doors were open, Containment Spray Systems I and 11 were

' considered to be " inoperable" as defined by the Technical Specifications. However, both systems would have perform 4d their designed functions as long as the structural integrity of the torus remained intact.

(i.e. no potential leak paths develop which could cause water to enter the pump rooms).

Based on documented operator tours of the area, the maximum amount of time the water tight doors could have been open was eight hours.

Corrective Action

The pump room doors were closed and dogged shut ir.rediately upon discovery.

An engineering evaluation will be made to determine an effective way of controlling access through the torus / corner room water tight doors.
