Description of Event

On February 26, 1981, train A of the Containment Quench Spray Subsystem was taken out of service and declared inoperable to repair a leak on an isolation valve (2-QS-27) to indicator FI-QS-203. The following day, after train A was restored to operable status, train B was removed from service to perform maintenance and repair leaks in mechanical fittings and hoses. This is contrary to T.S. 3.6.2.1 and reportable pursuant to T.S. 6.9.1.9.b.

Probable Consequences of Occurrence Since only one train was removed from service at a time, and each train was restored to operable status within 72 hours8.333333e-4 days <br />0.02 hours <br />1.190476e-4 weeks <br />2.7396e-5 months <br />, the health and safety of the general public were not affected.

Cause of Event

Each train of the Quench Spray Subsystem was removed from service to perform maintenance which improved the reliability of the system.

Immediate Corrective Action

Mechanical fittings were taken apart, cleaned and refitted. Flexible hoses which were leaking were also replaced.

Scheduled Corrective Action 2-QS-27 will be replaced during the next refueling outage.

Actions Taken to Prevent Recurrence

No additional action is required to prevent recurrence.
