UNSEALED CONDUlT WITH NO FIRE WATCH 1.

DESCRIPTION OF EVENT

With the unit at cold shutdown, construction personnel engaged in the performance of Design Change 78-34B, installed new conduit penetrations from the cable spread-ing room into the #1 cable tunnel. These penetrations were temporarily sealed in accordance with EMP-C-FP-51, however, after installation of additional cable to penetration A-1 the fire stop was not restored and a fire watch was not maintained.

This is contrary to Technical Specification 3 21.G.2 and is reportable in accord-ance with Technical Specification 6.6.2.b(2).

PROBABLE CONSEQUENCES AND STATUS OF REDUNDANT EQUlPMENT:

The functional integrity of the fire barrier penetration seals ensures that fires will be confined or adequately retarded from spreading to adjacent portions of the facility.

During the period when the seal was not functional with a fire watch not established, there were no fires in the affected areas; therefore, the health and safety of the public were not affected.

CAUSE

Construction personnel failed to restore the fire barrier seal when the additional cables were installed, and did not post a fire watch.

4.

IMMEDIATE CORRECTIVE ACTION

lne fire barrier was temporarily sealed in accordance with maintenance procedures.

5.

SUBSEQUENT CORRECTIVE ACTION:

The permanent seal for the fire barrier was installed.

6.

ACTION TAKEN TO PREVENT RECURRENCE:

The construction personnel have been re-instructed in the importance of maintain-ing a fire watch at all open fire barriers.
