Narrative

Report for LER 50-366/1981-084.

i On September 3,1981, with Unit II increasing power from 238 MWt, 2E41-N005, High Pressure Coolant Injection Steam Line Differential Pressure ' Switch, was found to actuate at +204.5" water Delta P.

Tech Specs Table 3.3.2-2 requires actuation at </= +196" of water Delta P.

Redundant switch 2E41-N004 was operable and in tolerance.

Plant operation or the public's health and safety were not affected.

This is a repetitive event as last reported on Reportable Occurrence Report No. 50-366/1980-141.

The cause of this event has been attributed to setpoint drif t.

The instru-ment was immediately recalibrated per HNP-2-5202, Barton Model 288A Cali-bration, and returned to service. A design change has been submitted to replace this instrument with an analog trip system.

A follow-up report will be submitted upon completion of this corrective action.

Unit I uses this same type of instrument for this application.

A generic review revealed that in this application this type instrument has inherent probl ems.
