Description of the Event:

l' With unit one at cold shutdown, an unscheduled review of electrical drawings

. by the on shift training coordinator prompted the testing of the auxiliary j

feed pump automatic start circuitry. The automatic start feature of the motor driven auxiliary feedwater pumps on loss of normal feedwater pumps was found to be inoperable.

Failure of the automatic pump start was caused by scheduled electrical maintenance removing unpowered control relays from the auxiliary feed pump cubicles for 1-FW-P-3A and 3B.

This work was performed due to an incomplete design change with improper documentation in the 1972/73 time frame.

This event is contrary to Administrative Procedure 39 and reportable per T.S.

Probable Consequences and Status of Redundant Equipment:

1 The automatic start feature of motor driven auxiliary feedwater pumps on loss of normal feedwater pumps is designed in anticipation of low steam generator level.

Pump auto start for all other functions remained operable during this event.

Since the anticipatory auto-start of the auxiliary feed pumps is de-signed for at power feed' pump trips and the unit remained at cold shutdown, the i

health and safety of the public would not have been affected.

Cause

The event was caused by an incomplete design change with improper documentation.

Immediate Corrective Action

The immediate corrective action was to install Jumpers in the Auxiliary Feed Pump control circuitry to allow auto-start upon loss of normal feed pumps.

Subsequent Corrective Action:

i The circuit will be permanently returned to the original design.

Station drawings FE8BD, 8BQ and ESK 5K will be changed to indicate the original design.

6.

Action Taken to Prevent Recurrence:

None.

7.

Generic Implications:

This event is not Generic to unit two since the feature that allows both normal feed pumps to be in pull to lock without auto-start of the auxiliary feed pumps' was never installed.
