GPERATIONS

SUMMARY

- SEPTEMBER 1978 At the beginning of the report period, the unit was operating at approximately 69% power in an end of cycle reactivity coastdown with control rod withdrawal i

limited by the' Technical Specifications.

On September 4,1978, a turbine trip and reactor scram occurred when the feedwater master controller failed due to a ground in the level recorder.

The unit was returned to service on September 5 following repairs to the feedwater controls.

On September 15, 1978, the unit was shut down to commence refueling operations.

Refueling continues at the end of this report period.

Six (6) Reportable Occurrences were identified during the report period:

d R0 #78-15 occurred during the post scram period on September 4 when both isolation condensers isolated by actuation of their line break sensors.

R0 #78-16 was identified on September 14 and involves failure to inspect two (2) hydraulic shock and sway arrestors as required.

R0 #78-17 occurred on September 14 when the closed position torque switch on isolation condenser valve V-14-37. failed, tr,ipping.the. motor overloads.

R0 #78-18 occurred on September 16 when three (3) main steam isolation

valves failed to meet the maximum allowable leakage rate.

l RO #78-19 occurred on September 15 when two (2) reactor building to torus vacuum breaker valves malfunctioned during operability testing.
