UNIVERSITY OF UTAH - NUCLEAR REGULATORYCOMMISSION SECURITY INSPECTION REPORT NO. 05000407/2022202

Dear Dr. Sjoden:

From July 11 - 13, 2022, the U.S. Nuclear Regulatory Commission (NRC) staff conducted a security inspection at the University of Utah TRIGA Nuclear Reactor. The inspection results were discussed on July 13, 2022, with you and members of your staff. The enclosed report documents the inspection results.

The inspection examined activities conducted under your license as they relate to security, compliance with the Commission rules and regulations, and with the conditions of your license.

The inspector reviewed selected procedures and records, observed activities, and interviewed personnel.

Based on the results of this inspection, the NRC has determined that one Severity Level IV violation of NRC regulatory requirements occurred. The violation was evaluated in accordance with the NRC Enforcement Policy. The current Enforcement Policy is included on the NRCs website at https://www.nrc.gov/about-nrc/regulatory/enforcement/enforce-pol.html. The violation is cited in the enclosed Notice of Violation (Notice) and the circumstances surrounding it is described in detail in the subject inspection report. The violation is being cited in the Notice because it constitutes a failure to meet regulatory requirements that has more than minor safety significance and the licensee failed to identify the violation.

You are required to respond to this letter and should follow the instructions specified in the enclosed Notice when preparing your response. If you have additional information that you believe the NRC should consider, you may provide it in your response to the Notice. The NRC review of your response to the Notice will also determine whether further enforcement action is necessary to ensure compliance with regulatory requirements.

When separated from enclosure, this document is decontrolled. In accordance with Title 10 of the Code of Federal Regulations (10 CFR) 2.390, Public inspections, exemptions, requests for withholding, a copy of this letter will be available electronically for public inspection in the NRC Public Document Room or from the NRC's document system (Agencywide Documents Access and Management System (ADAMS)).

ADAMS is accessible from the NRC Website at https://www.nrc.gov/reading-rm/adams.html (the Public Electronic Reading Room). However, because of the security-related information contained in the enclosures, and in accordance with 10 CFR 2.390(d), a copy of the enclosures will not be available for public inspection.
