(B) A list of all change in station design or operation, tests, and experiments made in accordance with Subsection 3.1 which involved a potentially significant unreviewed t - environmental issue, (C) -A list of non routine reports submitted in accordance . with Subsection 5,4,2. (D) Any results and/or assessments for the environmental monitoring. programs described in Subsection 2 0 of this EPP which were' submitted to the respective regulatory agencies during the annual reporting period. The following provides the Illinois Power Company response to each listed item. A, Listing of EPP noncompliances and corrective actions: I 1)' Noncomnliance: A total suspended solids sample-obtained March 12, 1990 from outfall 008 yielded 93 ppm total suspended solids-and resulted'in a 20 day average of 56 ppm,

Heavy' rainfall during that period is suspected to have caused excessive area runoff and flooding which resulted

Station does not_ currently contribute to this stream. , t 2)I Noncomnliance: c on June 12, 1990, a sample obtained from Outfall 002a did not contain detectable chlorine, Corrective Action: The tubo containing the chlorine tablets was placed upside down forcing the sewage flow around it, thereby not allowing the chlorine to dissolve in the slurry. Upon being informed of this condition, Radwaste placed the tube in a chlorinating position. Analysis on 6-13 90 resulted in 0.5 ppm chlorine.

On June 20, 1990, electrical power was lost to the fiume building at Outfall 002 from 0115 1340 which resulted in an inability to continuously monitor temperature during that time period, Corrective Action: A grab temperature during that time period was read as 94*F, The maximum temperature for that day was 95'F
- 4) Noncomo11ance:
On July 25, 1990, a grab sample collected at outfall 005 (Diesel Generator Area Oil-Water Separator) Contained 60 ppm of 011 and Grease. This resulted in a 30 Day Average of 17,3 ppm 011 and Grease, The previous three grab samples taken during the month of July contained'1,7 ppm, 5,1 ppm, and 2,5 ppm, respectively, A subsequent grab sample that was collected on August 1, 1990, contained 3,5 ppm of 011 and Grease, Corrective Action: The initial investigation of the Noncompliance identified malfunctioning outfall 005 sump pump level / shutoff float switch. The float switch has been repaired in accordance with the Clinton Power Station corrective maintenance program,
- 5) Noncomoliance:
The Total Suspended Solid Composite sample collected for Outfall 002a (Sewage Treatment plant Ef fluent) on September 6, 1990 was inadvertently disposed of without being analyzed, This discrepancy was not identified until September 13, 1990, which resulted in a missed weekly sample, The technicians, who collected the sample mistakenly entered the initial data for Outfall 002a sample into the receiving logbook indicating the Analysis had been s

started. The chemist, who performs sample analysis, reviewed the log book entry and thought the analysis had been completed. The sample was then marked for disposal. Corrective Action: The chemist has been counseled on the importance of reviewing the laboratory receiving logbook for valid sample Analysis results before marking the samples for disposal. Additionally, the Central Laboratory Staff received a briefing on the importance of accurately entering all data in the NPDES receiving logbook. l l

On September 10, 1990, the Circulating Water System was chlorinated from 0845 and 1030 and again from 1315 to , 1405 for a total of 2 hours and 35 minutes. The 2 hour per day chlorine injection time limit was exceeded by 35 minutes. However, the continuous chlorine detectable monitoring recorder indicated less than .001 ppm detectable chlorine. Therefore, the detectable chlorine concentration limit of .2 ppm was not exceeded. Corrective Action: The involved operations personnel have been instructed on the 2 hour per day chlorine injection time limit, Additionally, Operations has posted a sign at the chlorination inj ection area. The sign states that sodium
- hypochlorite shall only be injected for 2 hours or less per day.
- 7) Noncomo11ance: !
On October 29, 1990, a grab sample collected at Outfall 004_(Transformer Area Oil-Water Separator) contained 28 ppm 011 and Grease. A subsequent sample collected October 31, 1990 contained 29 ppm 011 and Grease. The Lprevious, two grab samples taken during the first and second weeks in October contained 3,3 ppm and 7,2 ppm 011 and Grease, respectively. This resulted in a 30 day Average of 16.9 ppm-Oil and Grease. Additionally, the grab sample collected on November 2, 1990, contained 3,9 ppm 011 and Grease,
- The investigation of the noncompliance at outfall 004 has determined the. source of oil and grease to be the result of transferring the contents of 011 Separator (OS) Number 2; into the outlet of 011 Separator (OS) Number 11, instead of the inlet to the oils separator, This error would 1 have discharge oil from outfall 004 directly to the lake, Corrective Action:
All Utility men associated with performing the transfer of' oil from either oil separator have been briefed on this noncompliance, Additionally, the oil separator covers have been color coded to indicate the inlet and outlet chambers. - 8) NoncompliaDCE:
The analysis result from the 12/13/90 sample biochemical oxygen demand (BOD) taken from outfall 002a (Sewage Treatment Plant Effluent) was determined to be invalid due to an incorrect dilution factor being used. The analysis results for this outfall have been consistent since it began operation so the Analysis was set up as is normally done. When the analysis result was higher than anticipated, the dilutions used for the Analysis were determined to be incorrect. Corrective Action: The exact caused of the increased BOD5 -is not known but was suspected-to be associated with flow changes't.t the sewage treatment plant during the compositing period. Other BOD $ analyses taken on 12 5, 12 19, and 12 27 showed- 7,10, ar.d 10 ppm respectively. This noncompliance was deemed to an isolated incident with no further corrective action being necessary. 9)- Noncomplianca:
-A sample taken from outfall 004 (Transformer Area 011-Water Separator) on 12 18 90 contained 22 ppm oil and grease. A sample taken from this-outfall on 12 31 90 contained.3.2 ppm and the two previous samples taken on 12 3 90 and 12-10 90 contained 1.1 and 7.9 ppm-oil and grease respectively.
A grab sample taken from outfall 005 (Diesel Generator Area Oil Water Separator) on 12-31-90 contained 42 ppm oil and grease. This result, when taken with the other results for December (1.3 ppm on 12 5 and 5.0 ppm on 12-
- 29) gave a 30 day average concentration of-16.1 ppm, Corrective Action:
The cause for the December, 1990 noncompliances resulted from maintenance actions to clean the separators and replace the baffle plates in the separator. The water discharSe pump was left on too long and oil / grease was taken into the pump and discharge line. When this pump was used again, the residual material was flushed on through the discharge piping. Actions to prevent recurrence include more attention to separator operation and better verification of a clean pump and discharge line. The cause for the noncompliance with Outfall 004 has not been determined despite walking down the inputs for this oil / water separator and checking the sept.rator itself. The station has closely observed the etfluent results to ensure that the separator is operating correctly. A maintenance work request has been initiated to clean this separator and check the baffles.

- 10) Noncomollance:
The Total Suspended Solids concentration (TSS) for the Ultimate Heat Sink Dredging Disposal Facility Outfall obtained on December 5, 16. and 30 yielded 120 ppm, 39 ppm and 66 ppm, respectively, This resulted in a 30 day
- TSS overage of 29.1 ppm for the month of December.
Corrective Actions: l The Noncompliance has been attributed to rain storms, high winds and wave action causing erosion of the Ultimate Heat Sink Dredging Disposal Facility berm / dike. The Dredging Operation was determined to not be attributing to high TSS. Clinton Power Station installed the stop log in the discharge tower on December 18. This action reduced the discharge flow rate from about 16 cubic feet per-second (cfs) to less than 2 cfs. This reduced flow rate was as low as could be achieved. It mainly consisted of Icakage between the stop. logs. This flow was steadily declined and will continue to decline. Additionally, Clinton Power Station has inspected the Disposal Facility berm / dike for erosion. The areas which-exhibited high erosion have been corrected by the placement of rip rap. _ No other. actions are presently planned to prevent. future problems due to natural causes.

.C. The following-is a non routine report submitted in accordance with Subsection 5.4.2:
On November'6, 1990, Illinois Power Company reported _a discharge of oil to Clinton Lake. The source of the release
- was disturbance-of the lake bottom resulting from the dredging operation to remove accumulated silt from the Ultimate Heat Sink in Clinton Lake. At approximately 4:13 p.m., oil was
-observed coming to the surface above the cutter head. An
- estimated 25-to 50 gallons of oil came to the' surface. Wind-and wave _ action spread the oil sheen to shoreline areas north and northeast of the dredging operation. Some oil was also pumped by the dredge to the dredging disposal facility. Oil was reported to have stopped coming to the surface at 5:30 p.m. -

Immediate Corrective Action and/or Counter Measures Taken: (a) Dredging operations were stopped and the cutter head was raised and inspected to ensure that-there was not a ; hydraulic leak. No leak was found. ] (b) 011 booms and absorbent materials were set out during the i night of November 6th into the morning of the 7th in all areas where an oil sheen was observed. O. H. Materials, Inc. was called in to assist with containment of the spill. (c) Notified Environmen:a1 Affairs Department for further notification to the National Response Center (Incident i No. 46660) and Illinois Emergency Services and Disaster-Agency (Incident No. 903285), (d) A sample of the liquid was sent to EMS Laboratories I (Indianapolis, IN.) for identification. EMS confirmed that the oil was diesel fuel. (e) The location of the oil source was surveyed so that the location could be found again at a later date. Illinois Power collected bottom solid samples and found signs of diesel fuel present in the top layer of the original bottom solids (beneath the silt). Diesel fuel was evident when the clumps of solid were broken apart. (f) A diver tried to find a container (e.g. drum, pipe, tank, etc.) on/in the soils that might have held the oil. No containers were found. (g) No environmental-impact was noted after the cleanup of the release. Additional Preventative Measures Taken or-Planned to Minimize the Possibility of Recurrence: (a) The Dredging Barge was moved past the location of the spill by 100 feet before the dredging operation was
-resumed.
(b)' John Mathes and Associates were called to conduct a geophysical survey in the area of the oil source to look for metal objects in the lake bottom which may contain oil, Core samples from the lake bottom will be collected to determine the horizontal and vertical size of any subsurface pockets of oil.
' (c) The results. of the John Mathes surveys (diver -aearch, underwater radar, and core sampling) yielded no further information. No.other signs of-oil contamination were found. No further investigations or corrective actions arc planned.

- 1. A further report on Clinton Lake Naegleria fowleri Monitoring and Clinton Lake temperature data was submitted on August 21, 1990. Additional 1990 temperature data was provided January 8, 1991.
There were nine pcaitive samples of N. fowleri (six bottom samples and three surface water samples). Out of 176 samples collected between June to October 1989. The warmest lake water temperature near Mascoutin Beach was 88.6'F on September 6, 1990. The complete information is provided in Attachments A and B.
- 2. The Clinton Lake segmentation survey for 1990 is provided in Attachment C. While the survey measured the 1990 depths near the Parnell Road Bridge and the DeWitt County Highway 15 Bridge, the Ultimate Heat Sink was dredging during the fall of 1990.
- 3. By the fall of 1989, silt accumulation had reduced the reserve volume capacity of the Ultimate Heat Sink to about 13%. Illinois Fower acquired Illinois Environmental Protection Agency permits 1990 EA 0172 and 1990 EA-0172 1, Illinois Department of Transportation permit 20129, and Army Corps of Engineers Permit 18827Z in order to dredge this accumulated silt from the Ultimate Heat Sink during the fall of 1990. The reserve capacity upon completion of the dredging was 83%. Attachments D and E provide information on the Archaeological surveys done in preparation for the dredging.

We are continuing our monitoring program with Dr. Huizinga in 1990 and will provide a copy of the results when they are available. I l l Clinton Lake Naegleria fowleri Monitoring Program 1989 Monitoring Report and 1983-1989 Program Summary Purpose This report summarizes the 1989 Naegleria fowleri monitoring program and water temperatures for Clinton Lake. It also presents a brief summary of the monitoring program data from 1983 through 1989. Methods and Materials Collection and analytical methods for 1989 were similar to those reported in 1988 by Dr. Huizinga in the
" Survey of Clinton Lake for the Occurrence of Naegleria f owler i_" . Samples were collected from Temperatures eight sites (Figure were
- 1) and on six dates in 1989 (Table recorded for each duplicate surf ace water 1). and bottom (Table 1). Temperatures weresamples also collected at each site recorded on an hourly basis by a DataSonde continual recorder at three of the eight collection sites to characterize lake temperatures.
Data for the program summary from 1983 through 1989 were obtained from the reports and data sheets from all the researchers involved. Results and Discussion 1989 Results A total of 176 samples were collected in 1989 and Naegleria fowleri were isolated from nine samples ( 5.1 %) TTable 2). Six of the N. fowleri were isolated from bottom Five of the samples and three from surface water samples. N. fowleri isolates were from sites most likely to be The remaining four

1 l 1 Program Summary 1983-1989 In 1983 and 1984, N. fowleri were not isolated from i Clinton Lake by Dr. Tyn3all (Table 4). However, in 1983, 1 Dr. Tyndall did isolate another thermophilic pathogenic amoeba, Acanthamoeba lenticulata from Clinton Lake. In 1986, Dr. Wellings found one sample positive for N. fowleri l (Table 4). This sample gas collected in November when the i
. water temperature was 12 C. Dr. Huizinga also had one-positive sample for N. fowleri in 19g6 and it was collected in July at a water temperature of 30 C.
The Clinton Power Station (CPS) first began operations in April of 1987, power levels were below 15% until mid June, which limited thermal discharges to the lake. By the end of August, the plant reached 70% power and thermal , discharges approached normal operating levels. Dr. Lewis did not isolate N. fowleri from Clinton Lake in 1987. Dr. Huizinga isolated N. fowleri from two Clinton Lake gamples in 1987. Thgse sampTes were collected in July at 28 C and October at 32 C and represented 1.4% of his 1987 samples. Extreme meteorological conditions in 1988 and the nearly continual operation of the CPS resulted in some of the warmest temperatures Clinton lake is likely to experience. A thermal aodel predicted that even without any ' thermal effluents, the monthly averages for~ June, July, and August would have ranked as the highest, fourth highest, and highest in the past 34 years. The Kational Weather service indicated the rainfall in June. JU' y, and August of 1988 was the second lowest in the past 110 , cars. In addition, the plant was at nearly full power throughout the entire summer. Data from continual temperature recorders indicated the CPS thermal effluent had some influence on elevated lake temperatures throughout the first half of the cooling loop. Lake temperatures from the dam (Site 10) back to the plant intake were not affected by plant operations. In 1988, Dr. Huizinga isolated N. fowleri from 21 of ! the 400 samples' (5.3%) collected (TaEle 4). Thirteen of the twenty one isolates were from bottom samples. All isolates were from sites which received at least some thermal influence from CPS. Maximum daily lakg temperature where the discharge enters the-lake was 42.2 C. -Isolates fgr N. fowleri were obtained at temperatures ranging from 18 t3~ l 38"C. I Dr. Huizinga's sample sites and analytical methods were similar'between 1986 and 1989. This provided the most comparable data base over this period and was the basis for l the following comparisons.
s Samples positive for N; fowleri were lowest (0.8%) in 1986 and highest- (5.3%) in 1988 (Figure 2). Samples positive for non-pathogenic Naegleria species appeared to vary inversely _with N. fowleri. The number of samples positive for N. fowleri at each lake site suggests N. fowleri were more prevalent at sites closest to-the thermal 31scharges (Figure 3). The highest number of N. fowleri isolates were from Site 4, where the thermal discharges enter Clinton Lake. }Iowever, Site 4 also received the highest sampling effort and the percentage of samples positiv,e for N. fowleri were higher at sites 5, 13D, and D2. The number oT~non-pathogenic Naegleria species show no definite pattern by site. Samples g were collected at temperatures which ranged from 2g to 46 C, with the majority being collected between 22 and 32 C (Figure 4). Naegleria fowler 1 were isolated from
- temperatures ranging from 18 to 38"C. The percentage of samples positive for N. fowleri were generally higher at the warmer temperatures (Figure 5). 1 1990 Program Update l The monitoring program for 1990 is being conducted similar to the 1989 program. Two additional lake sites have a been added and an attempt will be made to quantify N.
fowleri abundance by the "most probable numbers" metEod. Dr. Huizinga is also working on a new= technique which would reduce the time required to isolate N; fowleri from lake samples.

s Manager-Licensing & Safety V-920 October 26, 1990 RE: Clinton Lake Sedimentation Survey Attached are the results of the Clinton Lake sedimentation survey for 1990. Annual surveys are required by the CPS Environmental Poport - Operating License Stage (Section 6.1.6.8.7). This survey was conducted by EAD-Diological Programs personnel according to Procedure EAD-FD0-1, Rev. 1 on October 19, 1990. Attachments include survey forms and sonar graph recordings conducted along north-south transects adjacent to the Parnell Road Bridge and the DeWitt County liighway 15 Bridge. Two replicate recordings were determined at each bridge.

PROJECT AREA: O NE corner: N 4447360 / E 342830 (Zone 16) SE corner: N 4446160 / E 342800 SE corner: N 4446120 / E 342080 NW eorner: N 4447360 / E 342110 Project description: The project area includes approximately 220 acres (89.06 ha) of cultivated ground southwest of the Clinton Power Station in DeWitt County, it is the proposed site of a Dredge Disposal Facility for the Ultimate Heat Sink of the power plant. Construction will also include an outtiow channel extending south into a small draw (see attached map). The affected ground is a rectangular area in six contiguous 1/4-l/4 sections in Sections 27 and 34; T20N!R3E. It is divided into seven lields (A G). Topography: Minimally dissected uplands with occasionallow rises or knots. Poor drainage with some prairie potholes in the northeast part of the project area. Small draws are present south and northwest of the project area. (Solls) Mainly Birkbeek sitt loam, with significant minor representation of Keomah silt loam. Rotetta sitt loam, and Sable silly elay loam. Drainage: Includes the divide area between Salt Creek (south) and North Fork (northwest) Lund use/ ground cover (include % sisibilit)): The entire project area is cultivated. Ground surface visibility is 10 to 90%, except the southern end of the outflow channel. This area is in pasture. Its ground surface visibility is less than 10%. Survey Ilmitations: Fields were not fall plowed. Puddled sediments and erop stubble obscured the ground surface. Crop rows had higher visibility. O l 1

I took advantage of rows with higher ground visibility. Upon discovery of an art! fact, intervals were reduced to 2 - 5 m to determine ifit was an isolated find or part of a larger assemblage, in which case site limits were determined. I collected all visible objects except unmodified rock and fire-cracked rocks. On historical sites, the presence of brick, rock, concrete, and clinkers was noted, but they were not collected.
- Time expended: 44 hours.
Sitm/ find spots located: Thirty nine sites or isolated finds were identified and documented. Attached sheets show the location of the project area, field numbers of sites / isolated finds, higher and better drained soils (hatched), and more detailed maps of individual fields walked. IAS site forms are attached for all field loci. Table I shows the correspondence between tield locus numbers and lilinois , State Museum site numbers. Cultural material: See attached summary table and individual site forms. Seven sites yielded temporally diagnostic artifacts Inid!np: Locus 1 produced a Late Woodland point: Locus 10 an Early Archaic point; Locus 25 a Terminal Archale Merom(?) point; Locus 27 a Late Woodland / Mississippian triangular point; Locus 28 a Middle Archa!c side notched Ni nt; and Locus 30 another Termlnal Archaic Merom(7) point. Locus 39 consisted of an isolutM Late Woodland / Mississippian point. Early Archaic Thebes/ Dovetail cluster and Kirk like points were collected by a local relle hunter (Les Trotter, DeWitt) from Locus 27. Historical materials were recovered from Locus 3,23,26, and 34. Locus 3 is an unidentified
- scatter of whiteware, not associated _with a habitation / household. Locus 23 corresponds to the location of a house on the 1875 and 1915 plat maps. The same is true for Locus 34. Locus 26 corresponds to -
the location of a school (No. 6) on the 1915 and 1945 plat maps. A variety of non diagnostic cultural debris, consisting mainly of flakes and cores, was recovered from the other sites. Locus 6 produced high densities of material, relative to all other prehistoric sites. (Curated at): Illinais State Museum Research and Collections Center
- Collection techniques: At prehistoric sites I collected all v!Mb!stifacts with the exception of unmodified rock and fire-cracked rock. I examined these objects in the fielu, but did not collection them. On historical sites, I collected a grab sample of ceramie, glass, and metal artifactt ' Rock,
- brick, field tiles, concrete, clinkers and glass jar lids were noted but not collected.

COMMENTS: Sites near, but not affected by, con.,truction should be clearly identified with stakes or surveyor flags and avaided. l I recornmend no further work for the remaining prehistoric sites. The historical habitation sites are not significant in terms of regional research, and no standing architecture was observed. The school site (Locus 26) is interesting, but the artifact density is very low and no architecture is extant, in addition, its use is apparently confined to the 20th century. The rest of the sites consist of very low density scatters of prehistoric or historic debris, or isolated prehistoric finds.
