Dominion North Anna Power Station CONTROL ROOM JOB PERFORMANCE MEASURE EVALUATION OPERATOR PROGRAM INITIAL CONDITIONS A Unit 1 Feedwater suction line break has caused a loss of all Main Feedwater.

Unit 1 has tripped from 100% power.

All Auxiliary Feedwater Pumps are unavailable.

1-FR-H.1, Response to Loss of Secondary Heat Sink, was initiated and has been completed through Step 13.

INITIATING CUE You are requested to continue performance of 1-FR-H.1, beginning at Step 14.

(An extra Operator will be on the Control Board to address annunciators not associated with the task.)

TASK STANDARDS Given 1-FR-H.1, RCS bleed and feed is initiated by placing PRZR heaters in PTL, manually actuating SI, opening a PRZR PORV and Reactor and PRZR Vent valves.

Dominion North Anna Power Station CONTROL ROOM JOB PERFORMANCE MEASURE EVALUATION OPERATOR PROGRAM READ THE APPLICABLE INSTRUCTIONS TO THE CANDIDATE Instructions for Simulator JPMs I will explain the initial conditions, and state the task to be performed. All control room steps shall be performed for this JPM, including any required communications. I will provide initiating cues and reports on other actions when directed by you. Ensure you indicate to me when you understand your assigned task. To indicate that you have completed your assigned task return the handout sheet I provided you.

Instructions for In-Plant JPMs I will explain the initial conditions, and state the task to be performed. All steps, including any required communications, shall be simulated for this JPM. Under no circumstances are you to operate any plant equipment. I will provide initiating cues and reports on other actions when directed by you. Ensure you indicate to me when you understand your assigned task. To indicate that you have completed your assigned task return the handout sheet I provided you.

  • 1-SW-MOV-108B, SW Supply to Component Cooling Hx (GREEN LIGHT LIT)
  • 2-SW-MOV-208A, SW Supply to Component Cooling Hx (GREEN LIGHT LIT)
  • 2-SW-MOV-208B, SW Supply to Component Cooling Hx (GREEN LIGHT LIT).

Simulation / Verbal As the SRO, direct the Operator to operate the Unit 2 MOVs located on Cue(s) the Unit 2 Panel.

Notes/Comments A&B valves are in series so it is only critical that at least ONE valve on each Unit is closed (108A or 108B AND 208A or 208B).

Notes/Comments These MOVs are de-energized. Operator may call an operator to close the breakers so he can check its board position.

Booth Operator will close the breakers and call back to report that the breakers are closed.

Standards Operator verifies the following valves are closed:

  • 1-SW-MOV-115A, Aux SW Pump Disch A Header Isol Valve (GREEN LIGHT LIT)
  • 1-SW-MOV-118, Aux SW Pump Disch Cross Connect Valve (GREEN LIGHT LIT)

  • 1-SW-MOV-123A, Header 4 Bypass SW Reservoir Isol (GREEN LIGHT LIT)
  • 1-SW-MOV-123B, Header 3 Bypass SW Reservoir Isol (GREEN LIGHT LIT)

Notes/Comments: Need to depress and hold the Pushbutton.

Evaluators Operator may elect to direct the Unit 2 OATC to perform all of Step 3 of 0-AP-12, Note Attachment 3, rather than directing each individual sub-step, this option is acceptable, and IF used, THEN MARK steps 3a through 3e SAT, AND proceed to step 4.

7 Ensure Unit 2 Service Water Valves to Recirc Spray Heat Procedure Step Exchangers are closed. Att. 3 Step 3a SAT [ ] UNSAT [ ]

Standards Operator directs the Unit 2 OATC to ensure the following valves are closed:

  • 2-SW-MOV-201A, A SW Supply Header To RSHX Isol Valve (GREEN LIGHT LIT)
  • 2-SW-MOV-201B, A SW Supply Header To RSHX Isol Valve (GREEN LIGHT LIT)
  • 2-SW-MOV-201C, B SW Supply Header To RSHX Isol Valve (GREEN LIGHT LIT)
  • 2-SW-MOV-201D, B SW Supply Header To RSHX Isol Valve (GREEN LIGHT LIT)

Notes/Comments Booth operator will respond as the Unit 2 OATC and will report back that the MOVs are closed.

Evaluators Notes:

The Reset switches are similar to a typical Lockout Relay. They are latched in the RESET position and when actuated a coil is energized that releases the latch. Once the latch is released, spring force rotates the switch ~ 45° to the TRIP position (changing the state of the switch contacts) and maintains it there until reset by the operator, by rotating the switch until it re-latches.

Note: Prior to resetting the Trip-Reset switches, the Local-Remote switches at the Auxiliary Shutdown Panel MUST be placed in REMOTE. Evaluators Notes:

The Reset switches are similar to a typical Lockout Relay. They are latched in the RESET position and when actuated a coil is energized that releases the latch. Once the latch is released, spring force rotates the switch ~ 45° to the TRIP position (changing the state of the switch contacts) and maintains it there until reset by the operator, by rotating the switch until it re-latches.

Note: Prior to resetting the Trip-Reset switches, the Local-Remote switches at the Auxiliary Shutdown Panel MUST be placed in REMOTE. All control room steps shall be performed for this JPM, including any required communications. I will provide initiating cues and reports on other actions when directed by you. Under no circumstances are you to operate any plant equipment. I will provide initiating cues and reports on other actions when directed by you. INITIAL CONDITIONS The Control Room has been evacuated due to a potential toxic gas release.

Both units were tripped prior to leaving the control room.

All IRPIs on Unit 2 indicated less than 10 steps except for the following:

  • G-3 16 steps
  • D-10 18 steps
  • M-4 25 steps 2-AP-20 has been completed up to and including step 12f.

Operators are stationed at the Auxiliary Shutdown panel.

The C Boric Acid Storage Tank is in service to UNIT 2.

INITIATING CUE You are requested to emergency borate Unit 2 per 2-AP-20.

Standards Attachment 3 is initiated to determine Total Equivalent Stuck Rods = 2 A minimum of 50 minutes flow or 30% BAST level is required.
