' Washington, DC 20555 DON TRIGA RESEARCE REACTOR - DOCIET 50-264 REPORTABLE OCL M on December 20, 1989 the wide-range log channel failed while the reactor was being' taken to power. The manual scram was immediately activated and the reactor was safely secured.
This failure was-caused by a poor connection in a rotary switch in the wide-range log channel (calibrate switch). The switch was repaired, the log channel.was successfully tosted by comparison with the independent linear power channel, and the reactor was returned.to operation the same day.
Similar failutes occurred December 3, 1984, January 7, 1985, and November 12, 1985. The actual cause of these faf! res was identified after the November 1985 event. The circuit has performed properly for four years since that event.
That switch is operated daily as part of the checkout procedures it appears that repeated use of the switch leads to the poor contact condition and to the subsequent failure during operation.
The switch will now be inspected at the time of the annual fuel inspection, starting in January 1991, and will be repaired or replaced if further evidence of malfunction is discovered.

Page 2' A request for capital authorization to replace the entire control console will be initiated early in January 1990, with the aim of installing a'new system during 1990. Such an installation will eliminate any further problems with this particular circuit.
