Simulator Scenario Outline Comments (9/25/12)

Comment Resolution Scenario 1, Event 6, 7 - Loss of CCW Revised leading to the reactor scram is ONE 1

Major event. Event 6 and 7 are not both Majors. Revise D-1.

Scenario 2, Event 5 - Any chance the No applicants could scram due to the loss of 2

two IRM channels? If so we should move right before the major.

Scenario 2, Event 7 and 8 - Same Corrected comment as the first scenario. The major event is the scram due to the 3 CRDM malfunction. This is really one event. The CST rupture would be its own event and count as the component failure.

Scenario 2, Event 8 - Is the un-isolable No recirc line break counted as the 4 component malfunction? Do the operators perform any functions to correct even though it is un-isolable?

Scenario 2, Event 9 - What is the major 5 electrical transient and the crew response?

6 Scenario 3, Event 1- Secure the Div. 1 DG - what is the failure?

7 Scenario 3, Event 6 - Loss of EST Corrected transformer cannot count toward Abnormal due to following the Major (1021, App. D, pg. 9) 8 Scenario 3, Event 7 - What is the crew response to ADS valve failure to open?

10 Scenario 4, Event 5 and 6 - CRDM Corrected malfunction leads to ATWS. This is one major. The CR drifts count as the instrument malfunction but the CRDM with ATWS is one event. Reduce major count by one.

11 Scenario 4, Event 8 - Critical Task Defined must be accomplished prior to reaching HCTL. Please define for Examiners. OBDI 202 - IOLE Process

12 Scenario 5, Event 1 - What is the Fixed failure with placing SSW A in Standby?

Event 1 counted as component malfunction.

13 Scenario 5, Event 3 - Any chance No applicants could scram plant because of the failure of two APRM channels?
