ÿAfailureofthehospital'sRadiationSafetyCommittee,whichoverseestheuseofradioactivematerialsatthe facility,toreviewtheboardcertificationsorpermitsof individualspriortopermittingthemtoworkasauthorized

users.ÿAfailuretouseradioactivematerialsasauthorizedby license.ÿAfailuretoinstructappropriatepersonnelintheapplicableregulationsandlicenseconditionspriortothe startoftheirduties.

ÿAfailuretomaintainrecords,asrequired,forsurveysandforwastedisposal.Atthepredecisionalenforcementconference,hospitalofficialswillhaveanopportunitytoprovidetheirperspective ontheapparentviolations,discusstheircausesandsafety significance,anddetailanycorrectiveactionsthathavebeen takeninresponse.NodecisiononwhethertheNRCwilltakeanyenforcementactionwillbemadeatthissession.Thatdecision willbemadebyNRCofficialsatalatertime,usuallywithin about30days.
