However,becausethesourcesbeingusedshiftedduringtreatment,thepatientreceivedalesserdose thanprescribedtothecanceroustissueandahigherdosethanprescribedtosurroundingtissues.A medicalconsultantcontractedbytheNRCtoreviewtheincidentindicatedthepatientwasexperiencing medicalcomplicationsthatmaybeattributabletothemisadministration.Basedontheinspection,theNRChaspreliminarilydeterminedthatthehospital'squalitymanagementprogramfailedtoincludeawrittenproceduretomeettheobjectivethateach administrationofnuclearmedicinebeinaccordancewithawrittendirective.ThedecisiontoholdapredecisionalenforcementconferencedoesnotmeanthattheNRChasdeterminedaviolationhasoccurredorthatenforcementactionwillbetaken.Rather,thepurposeisto discusstheapparentviolation,itscausesandsafetysignificance;toprovidethelicenseewithan opportunitytopointoutanyerrorsthatmayhavebeenmadeintheNRCinspectionreport;andto enablethelicenseetooutlineitsproposedcorrectiveaction.Nodecisionontheapparentviolationwillbemadeatthisconference.ThatdecisionwillbemadebyNRCofficialsatalatertime.
