| Description | Technical Specification 5.5.1.1 requires, in part, that written procedures be established, implemented, and maintained covering the activities specified in Appendix A, Typical Procedures for Pressurized Water Reactors and Boiling Water Reactors, of Regulatory Guide 1.33, Quality Assurance Program Requirements (Operations), dated February 1978. Appendix A, Section 3.f, requires procedures for maintaining containment integrity. Procedure SO23-5-1.8.1, Shutdown Nuclear Safety, Revision 19, documented the containment closure crew responsibilities to maintain containment integrity for various reactor coolant system conditions. Contrary to the above, on November, 23, 2008, a dedicated containment closure crew was not onsite for approximately 55 minutes when Unit 3 was in Mode 5, and reactor coolant system conditions required that containment integrity was maintained. This issue is documented in the licensees corrective action program as Nuclear Notification 200224995. The finding is of very low safety significance because the finding does not increase the likelihood of a loss of reactor coolant system inventory, degrade the licensees ability to terminate a leak path, add reactor coolant system inventory, or recover decay heat removal once it is lost |
