L-97-016Attachment1.FPLconcurswiththeviolation.2.ThecauseofthisviolationwastheineffectivetransferofaccountabilityfortheUnit1radiationmonitoringinstrumentationfromChemistryandHealthPhysicsDepartmentstotheI&CDepartmentpriortoensuringthattheaffectedI&Cpersonnelhadcompletedallappropriatetrainingqualifications.TrainingforthesubjectI&CpersonnelhadbeenscheduledtobeimplementedandwaspostponedbytheI&Csystemsupervisorduetoworkpriorities.AsubsequentverificationwasnotmadebyI&Csupervisiontoverifythatpersonnelhadbeenfullytrainedpriortoreceivingresponsibilityfortheaboveinstrumentation.A.ArevisedschedulewasimplementedforthecompletionofthespecifictrainingandqualificationrequiredforI&Cpersonnelresponsibleforperformingradiationmonitorcalibrationandtesting.TheabovetrainingforthesepersonnelwillbecompletedbyMarch31,1997,followingareviewoftheassociatedI&Cprocedures.B.Asupervisor'smeetingwasheldbytheI&CDepartmentheadtoaddressandemphasizetherequirementsforobtainingpriordepartmentheadapprovalofchangestoscheduledtrainingandensuringthatallpersonnelareproperlyandcurrentlytrainedtoperformthetaskswhichtheyareassigned.I&Cpersonnelperformingradiationmonitoringcalibrations,andfunctionalcheckswererequiredtoworkunderadditionalsupervisoryoversightpendingthecompletionoftheappropriatequalificationtraining.C.AppropriateTrainingpersonnelwerecounseledontheimportanceofinformingmanagementwhentrainingorqualificationrequirementsarenotbeingadequatelymet.4.A.TherequiredtrainingandqualificationofI&Cpersonnelresponsibleforperformingcalibrationandtestingofradiationmonitoringinstrumentationwill
