St.LucieUnit12DocketNos.50-335and50-389ReplytoNoticeofViolationInsectionReort95-17VIOLATION10CFR50AppendixB,CriterionV,Instructions,Procedures,andDrawings,asimplementedbyFPLTopicalQualityAssuranceReport(FPLTQAR1-76A),requiresthatactivitiesaffectingqualityshallbeprescribedbydocumentedproceduresandactivitiesshallbeaccomplishedinaccordancewiththeseprocedures.Contrarytotheabove,onDecember4-15,1995,threeexampleswereidentifiedinwhichactivitiesaffectingqualitywerenotaccomplishedinaccordancewithprescribedproceduresorinstructions.Example1:ProceduresAP0010432,NuclearPlantWorkOrders,revision87,andQI7-PR/PSL-1,ControlofPurchasedMaterial,EquipmentandServices,revision28,requirethatmaterialfielddedicationbeperformedasspecifiedonmaterialidentificationtagsandrelatedprocurementevaluations.WorkOrders95020550,95020551,95020552,and95020545,datedNovember27,1995,installedunqualifieddiodesonsafetyrelatedReactorCoolantSystem(RCS)VentSystemsolenoidvalvesV-1460,V-1461,V-1462,andV-1463.Thecommercialgradededicationtestingspecifiedonthematerialidentificationtagsandspecifiedonprocurementevaluation039431wasnotperformed.Example2:ProcedureIP-1102,In-StorageMaintenanceandShelfLifeProgram,revision0,requiresthatequipmentin-storagemaintenanceandshelfliferequirementsbeenteredintotheshelflifeprogram.Purchaseorders0007466and0007379,datedOctoberandDecember,1994,receivedregulatedpowersourcedevices.Thecommercialgradededicationevaluation036828includedarequirementtoenergizethedeviceseverytwoyearstomaintaintheshelflife.AsofDecember5,1995,thisrequirementwasnotenteredintotheshelflifemaintenanceprogram.Example3:FPLSupplierDeviationNoticeSDN-SL-95-034-5datedNovember21,1995,identifiedthatthefusesprocuredonPurchaseOrder00012173didnotmeetFPLtestrequirements.Engineeringevaluation040129providedinstructionsforresolutionoftheSDNwhichrequiredtheunqualifiedfusesbeplacedonholdforadditionalengineeringevaluationuponreceiptfromthevendor.Thefusesweresubsequentlyreceiptinspectedandmadeavailableforsafetyrelatedissueinthewarehousewithoutthespecifiedengineeringevaluation.

St.LucieUnit12DocketNos.50-335and50-389ReplytoNoticeofViolationInsectionReort95-17FPLRESPONSE1~CONCURRENCEANDREASONFORTHEVIOLATIONFPLconcurswiththeaboveviolation.1~ReasonforExample1oftheviolation:TherootcauseforthefailuretoperformtherequireddedicationtestingondiodesdescribedinExample1wascognitivepersonnelerrorbycontractorpersonnel.Thediodeswerelabelledwithmaterialidentificationtagswhichidentifiedthecomponentsaspartsrequiringadditionaldedicationtesting.Thecontractorpersonnelwhoreceivedthediodesfromthematerialwarehousedidnotunderstandthemeaningoftheidentificationtagswhichaccompaniedthematerialsandincorrectlyassumedthatallrequiredtestinghadbeencompleted.Acontributingfactorinthiseventwasthatthelevelofdetailincludedintheinitialtrainingofthecontractorpersonnelinvolvedinthiseventwasinsufficienttofullyaddresstheadministrativerequirementsrelatedtomaterialdedication.2~ReasonforExample2oftheviolation:TherootcauseforthefailuretoincludeshelfliferequirementsdescribedinExample2wasaninadequateinter-departmentalprocessfortransmitting,andverifyingthereceiptof,in-stockdispositions.Theprocessdidnotrequireconfirmationthatthein-stockdispositionhadbeenreceivedandthattheappropriateactionshadbeenimplemented.Asaresult,therequirementtoenterthesubjectpowersourcedevicesintotheshelflifeprogram,asrequiredbytheengineeringevaluationandapprovedplantprocedure,wasnotimplemented.3.ReasonforExample3oftheviolation:Theimmediatecauseforthefailuretocomplywiththerequirementsoftheengineering"hold"citedinExample3,wasasoftwareprogrammingdeficiencywhicheffectivelyalloweddataconcerningtheengineeringevaluationtogounidentified.FPLusesanelectronicdatabaseprogramtoidentifyengineeringholdsbyuseofacomputermessage,or"flag".

St.LucieUnit12DocketNos.50-335and50-389ReplytoNoticeofViolationInsectionReort95-17Therootcauseforthiseventwasthatthesoftwaretestingprocessusedinthisapplicationwasinsufficienttoidentifyanerrorwiththisparticularprogramfeature.A'reviewofthesoftwaretestingmethodologyshowedthatthespecifictestingperformedwouldnothaveexposedthetypeoferrorwhichoccurredinthiscase.2.CORRECTIVESTEPSTAKENANDTHERESULTSACHIEVED1~CorrectivestepstakenforExample1:a~FPLInstrumentandControlmaintenancepersonnelperformeddedicationtestingontheremainingdiodesofthesamestockcodewhichhadnotbeeninstalledintheplant,inaccordancewithprocurementevaluation039431.Operabilityoftheinstalleddiodeswasthenestablishedbasedonsatisfactorycompletionofthededicationtestingperformedonthediodesinstores,andthepostmodificationvalvetestingperformedfollowingtheinstallationofthesubjectdiodesintotheRCSsolenoidvalves.AlltestingwascompletedinaccordancewithengineeringdispositiontoSt.LucieActionReport(STAR)952133,onDecember13,1995.b.Thecontractorpersonnelinvolvedinthisevent,theirsupervisors,andothercontractorpersonnelrequiredtoreceivematerialsfromstoresforinstallationintheplant,weregivenadditionaltraining.Thistrainingemphasizedthelessonslearnedfromthiseventandreviewedtheadministrativerequirementsassociatedwithmaterialdedication.ThisactionwascompletedonJanuary18,1996,perSTAR960081A.2~CorrectivestepstakenforExample2:a0b.ThesubjectpowersourcedeviceswereenteredintotheshelflifeprogramasrequiredbythecommercialgradeprocurementevaluationandFPLprocedure.ThisactionwascompleteonDecember12(1995.Aself-assessmenttovalidatecurrentinventorybymeansofphysicalcomparisontothematerialcatalogdescriptionforallProcurementClassification(PC)-1,2and3materialwasconductedfromSeptember1,1995,untilDecember

St.LucieUnit12DocketNos.50-335and50-389ReplytoNoticeofViolationInsectionReort95-1722,1995.Thevalidationself-assessmentincludedareviewofshelfliferequirements.Theself-assessmentidentified75shelflifediscrepanciesoutofmorethan19,000stockcodesreviewed.Thediscrepancieswerecorrectedbyeitherin-stockreplenishment,databasecorrection,or.engineering-approvedshelflifeextensions.Theshelflifediscrepanciesidentifiedbytheself-assessmentwerecorrectedbyDecember22,1995.3.CorrectivestepstakenforExample3:a~ThesubjectfuseswereplacedonholdbyProcurementEngineeringpendingadditionalevaluation.Inaddition,plantstaffverifiedthatnoneofthesubjectfuseshadbeenusedinsafety-relatedapplications.b.FPLperformedaninvestigationofthegenericimplicationsresultingfromtheprogrammingerror.PotentialproblemswerereviewedatbothSt.LucieandTurkeyPointsites.Noadditionalmaterialdeficiencieswereidentifiedasaresultofthisreview.c.NuclearInformationServices(NIS)issuedasoftwarecorrectiontotheaffectedplantdatabaseprogramonDecember8,1995,perSTAR952127.3~CORRECTIVESTEPSTOAVOIDFURTHERVIOLATIONSCorrectivestepstoavoidfurtherviolationsasidentifiedinExample1:a~b.Acomprehensivereviewoftheprogrammaticcontrolsassociatedwithdedicationtesting,toincludeworkinstructions,training,andadministrativecontrols,willbeconductedtodeterminewhereadditionalimprovementscanbemadeinthisarea.ThisactionwillbecompletedbyApril30,1996.Thematerialtagsusedtoidentifycomponentswhichrequireadditionaltestingpriortouseintheplantwill'berevisedtoimproveadministrativecontrol.Aspartofthisaction,FPLwillreviewindustrybestpracticeinthisarea.This,actionwillbecompletebyMarch29,1996.4

4't.LucieUnit,12DocketNos.50-335and50-389ReplytoNoticeofViolationInsectionReort95-17c~TheprogramforinitialorientationandcontinuingcontractortrainingwillbemodifiedtoincludecoverageofappropriateSt.Lucieandindustryoperatingexperience.ThisactionwillbecompletedbyApril30(1996.2~CorrectivestepstoavoidfurtherviolationsasidentifiedinExample2:a~b.Theprocessforimplementingshelfliferequirementshasbeenchangedtoeliminatetheneedforamanualtransferofinformation.FPLNuclearMaterialsManagement(NMM)isnowusingacommondatabasesystemwithProcurementEngineeringwhichhasimprovedtheinterfaceprocessbetweenthetwoorganizations.Useofacommondatabasesystemprovidesafeedbackprocessforverificationofin-stockdispositions.FPLwillassesstheoverallprocessforidentifyingandimplementingmaterialshelfliferequirementstodeterminewhereadditionalimprovementscanbemade.Thisassessmentwillincludeareviewofindustrybestpracticeinthisarea.ThisactionwillbecompletebyApril30,1996.c~NMMiscurrentlyperformingadditionalmaterialsverificationtoconfirmthatallactionsrequiredasaresultofpreviousin-stockdispositionshavebeencompleted.ThisverificationwillbecompletebyApril30,1996.3~CorrectivestepstoavoidfurtherviolationsasidentifiedinExample3:a~Thiseventanditscorrectiveactionswerereviewedwiththeanalystsandprogrammersassociatedwiththesubjectdatabasesystem.Additionaltraining,whichreviewedcurrenttestingrequirements,wasconductedforNISpersonnelonJanuary12,1996.b.NISwillincorporateintotheSoftwareQualityAssuranceprogramtheadditionalrequirementtoidentifyandappropriatelytestany"criticalfunctionality"associatedwithasoftwareapplicationbeingdeveloped.Criticalfunctionalityreferstothoseattributesofasoftwareapplicationwhosefailuremaynotbereadilydetectableorobvious,andcouldresultin
