1990 ANNUAL REPORT PRIMARY SAFETY AND RELIEF VALVE CHALLENGES On July 26, | |||
: 1990, pressurizer power-operated relief valve (PORV) 1-PCV-444B was inadvertently opened by a reactor operator. | |||
The PORV was immediately reshut. | |||
Approximately 34 gallons of primary coolant was discharged to the Pressurizer Relief Tank during the cycling of the PORV. | |||
The event occurred while performing prerequisites for a surveillance test. | |||
The operator was to move the control switch for 1-PCV-444B from the automatic control position to the closed position but instead moved, the switch in the wrong direction to the open position. | |||
The event was reviewed with each of the operating shifts. | |||
