1990 ANNUAL REPORT PRIMARY COOLANT IODINE SPIKES During 1990, the primary coolant did not exceed 1.0 pCi/gram dose equivalent I-131 or 100/E-bar pCi/gram gross radioactivity as set forth in Technical Specification 3.4.8.

1990 ANNUAL REPORT PRIMARY SAFETY AND RELIEF VALVE CHALLENGES On July 26, 1990, pressurizer power-operated relief valve (PORV) 1-PCV-444B was inadvertently opened by a reactor operator. The PORV was immediately reshut. Approximately 34 gallons of primary coolant was discharged to the Pressurizer Relief Tank during the cycling of the PORV. The event occurred while performing prerequisites for a surveillance test. The operator was to move the control switch for 1-PCV-444B from the automatic control position to the closed position but instead moved, the switch in the wrong direction to the open position. The event was reviewed with each of the operating shifts.
