AAR 2011 McGuire Nuclear Station REP Exercise Appendix A: Scenario Summary This exercise will be a full station exercise with the Simulator Control Room, Technical Support Center (TSC), Operations Support Center (OSC), and the Emergency Operations Facility (EOF) staffed with players, controllers and evaluators. The Joint Information Center (JIC) and the Media Center will be staffed. The State of North Carolina and the counties of Catawba, Cabarrus, Charlotte-Mecklenburg, Gaston, Iredell and Lincoln are participating in this exercise and will be observed by FEMA representatives.

The exercise begins with the plant operating at full power on both units. The plant conditions are as follows:

Unit 1 is operating at 100% power. The 1B Diesel Generator (D/G) is out of service due to work being performed on the governor valve. It is expected to be returned to service in approximately 8 hours9.259259e-5 days <br />0.00222 hours <br />1.322751e-5 weeks <br />3.044e-6 months <br />.

At approximately 3 minutes after the crew assumes the watch the crew will experience a grid disturbance due to frequency instability caused by partial loss of the grid. The Control Room Supervisor will implement abnormal procedure (AP)-05 (Generator Voltage and Electrical Grid Disturbances). The frequency will drop to a value requiring the unit to be separated from the grid. After separation from the grid the Unit 1 main generator will be supplying all power requirements for Unit 1. Unit 2 will not be affected by the events on the 235KV grid. The Control Room Supervisor will implement AP-03 (Load Rejection). A loose part alarm will be received in the control room. Later in the scenario fuel damage will occur due to the loose part. This will be indicated by an increase in radiation monitors 1EMF51A and 1EMF5 lB.

Forty six minutes into the drill main condenser vacuum will degrade due to in-leakage occurring on a vacuum breaker. Condenser vacuum will degrade to a value that will require the crew to trip the Unit 1 reactor, which will trip the main turbine. The Control Room Supervisor will implement E-0 (Reactor Trip or Safety Injection). When the main turbine is tripped it will cause a loss of normal power to both 4160v essential busses. The 1A D/G will start and supply power to "A" train essential equipment. AP-07 (Loss of Electrical Power) will be performed concurrently with Emergency Management Guideline (EMG) E-0 as directed by E-0.

An Alert will be declared due to power being reduced to only one power source per RP/000 emergency action level (EAL) 4.5.A.2. RP-02 Alert will be implemented.

When conditions for site assembly are met, this onsite Protective Action will be demonstrated. When conditions for a site evacuation are met, this onsite protective action will be simulated.

AAR 2011 McGuire Nuclear Station REP Exercise Control Room Supervisor will implement ECA 0.0 (Loss of ALL AC power).

The TSC/EOF Emergency Coordinator will declare a Site Area Emergency (SAE) due to the loss of all AC power per RP 000 EAL 4.5.S.1.

Equipment required to stabilize the plant will be started following restoration of power to the bus..

Approximately 3 hours3.472222e-5 days <br />8.333333e-4 hours <br />4.960317e-6 weeks <br />1.1415e-6 months <br /> 35 minutes into the event a reactor coolant leak inside containment occurs resulting in a loss of subcooling. This will cause containment 1 pressure to increase to approximately 12 psig. Annulus ventilation starts as required and annulus pressure cannot be maintained negative as designed.

Approximately 3 hours3.472222e-5 days <br />8.333333e-4 hours <br />4.960317e-6 weeks <br />1.1415e-6 months <br /> 56 minutes containment pressure begins to decrease rapidly indicating that a containment breach has occurred into the annulus. The annulus ventilation discharges to the unit vent which results in an offsite release of radioactive gases. 1EMF 51A and 1EMF 51B increase to approximately 39R/hour.

I Approximately 1210 the EOF Emergency Coordinator declares General Emergency (GE) based on EAL 4.1 .G. 1 (Loss of All Three Barriers). This is based on a rapid decrease in containment pressure following initial increase, a loss of subcooling following the loss of cooling accident (LOCA), and 1EMF 51A & B exceeding 31 R/hour 2-4 hours after unit I shutdown.

At this point Siren and Emergency Alert System (EAS) activation will be simulated upon l the GE.

Time Event I 0800 The plant is operating at full power. IB emergency D/G is out of service (OOS) for repair to governor valve.

0803 Grid disturbance resulting in lowering of grid frequency requiring separation from the grid 0816 Loose part annunciator received in the control room 0846 Main condenser vacuum begins to degrade to a level requiring the reactor and main turbine to be tripped.

0950 OSC and TSC activated. EOF may be operational.

1013 1A D/G trips due to low lube oil pressure resulting in a loss of all AC power.

1028 TSCJEOF Emergency Coordinator declares Site Area Emergency based on EAL 4.5.S. 1 (Loss of All Offsite Power and Loss of All Onsite Power to Essential Busses) 1045 State and Counties notified of Site Area Emergency via ENF using SSS. NRC notification follows immediately but no later than one hour after Site Area Emergency declaration.

1134 A primary system LOCA into containment occurs resulting in loss of subcooling.

1156 A leak from containment to the annulus occurs. 1EMF 51A & B exceed 31 R/hour.

1210 EOF Emergency Coordinator declares General Emergency based on EAL 4. I.G.2 (Loss of Any Two Barriers AND Potential Loss of the Third).

1225 State and Counties notified of General Emergency via ENF using SSS. NRC notification follows immediately but no later than one hour after General Emergency declaration. Protective Action Recommendations (PARs) are communicated PARS:

Evacuate: Zones A, B, C, D, L, M, N, 0, R Shelter in place: Zones E, F, G, H, I, J, K, P, Q, S 1400 Drill terminated.

" State and local response personnel may pre-position staff at their EOC.

" Utility will provide a liaison to the State emergency operations center (EOC),

Western Branch Office, Catawba County EOC, Gaston County EOC, Iredell County EOC, Lincoln County EOC and Charlotte-Mecklenburg County EOC.

• A State or county escort will accompany Federal Evaluators to Out of Sequence demonstrations.

• A State Controller will be located in the State EOC, Catawba County EOC, Gaston County EOC, Iredell County EOC, Lincoln County EOC and the JIC.

• Area Coordinators will deploy to the Western Branch Office during the exercise.

" Exercise participants will have the opportunity to remediate and re-demonstrate exercise criterion immediately upon identifying any errors with the approval of the Federal Evaluator during field demonstrations.

the REP Program pursuant to the regulations found in 44 Code of Federal Regulations (CFR) 350, 351 and 352. 44 CFR 350 codifies 16 planning standards that form the basis for 1

radiological emergency response planning for licensee, State, tribal, and local governments impacted by the emergency planning zones established for each nuclear power plant site in the United States.

an annual MSD. The MSD involves a simulated contaminated injured individual and contains provisions for participation by local support service agencies (i.e., offsite ambulance and medical treatment facility). The drills enable the emergency medical service and medical facility staff to 1

demonstrate proper care of that contaminated, injured, or exposed individual. The drills focus on contamination control and decontamination techniques. 1 A MSD was evaluated on August 10, 2011 and included evaluations of Charlotte-Mecklenburg 1

5. FEMA MSD Objectives and Capabilities Capabilities-based planning allows for exercise planning teams to develop objectives and observe. outcomes through a framework of specific action items that were derived from the Target Capabilities List (TCL). The capability listed below form the foundation for the organization of all FEMA Region IV REPP objective and observations in this exercise.

" Objective: The facility/offsite response organization (ORO) has the appropriate space, adequate resources and trained personnel to provide transport, monitoring, decontamination and-medical services to contaminated injured individuals.

" Capability: Triage and Pre-Hospital Treatment Triage and Pre-Hospital Treatment is the capability to appropriately dispatch emdrgency medical service (EMS) resources; to provide feasible, suitable, and medically acceptable pre-hospital triage and treatment of patients; to provide transport as well as medical care en-route to an appropriate receiving facility; and to track patients to a treatment facility.

Additionally, this capability is linked to several corresponding activities and tasks to provide additional detail. Based upon the identified exercise objectives, the associated activities for the above capability are:

• Direct Triage and Pre-Hospital Treatment Operations;

" Activate Triage and Pre-Hospital Treatment;

" Transport; and

" Provide Treatment The MSD involved a simulated cOntaminated injured individual and contained provisions for participation by local support'service agencies(i.e., ambulance and offsite medical treatment facility). It demonstrated that procedures for transporting contaminated, injured or exposed individuals to the appropriate medical facility could be implemented. The drill purpose was to enable the medical facility staff to demonstrate proper care of a contaminated, injured or exposed individual. The focus of the drill was contamination control measures, not medical protocols per se with the exception being a modification of contamination control procedures and decisions based upon an urgent medital condition.

6. Scenario Summary:

Jeffrey Cardwell was developing a new testing procedure in, the laboratory of Cardwell Enterprises Labs. He had to wait for a fellow researcher to complete their project before the exhaust hood would be available for his use. AAR 2011 McGuire Nuclear Station REP Exercise made available and Mr. Cardwell got started with his project involving the use of Cesium 137 in powdered form. 3 Unknown to Mr. Cardwell, the earlier experiment had left an explosive mixture of gases in the work area under the hood due to a malfunctioning exhaust fan. When the gas mixture reached a shorted wire there was a small explosion and flash fire. At the time the explosion occurred Mr. Cardwell's arms were working with the Cesium 137 under the hood. The radioactive powder was spilled and blown out into the immediate area in front of the exhaust hood.

Mr. Cardwell fell back into a lab table knocking him unconscious for a short period of time. He received cuts, abrasions, and first degree burns to his arms and some bruising to 3

his mid section as a result of the explosion. He had radioactive contamination in several areas due to his proximity to the event when the Cesium was spilled and spread.

Hearing breaking glass, his fellow employees investigated to find Mr. Cardwell needing medical assistance and could not get him to acknowledge them. Knowing that the area was possibly contaminated they did not enter the room and waited for assistance at the I

door.

Note: For the purpose of this drill the actual location for the patient to be picked up was at the MEDIC Headquarters Training Facility located on Statesville Road in Charlotte. The patient was then transferred to CMC-University via the responding EMS/MEDIC Unit.

7. Analysis of Capabilities I Contained in this section are the results and findings of the evaluation of the MSD in support of McGuire Nuclear Station.

After receiving a call for assistance MEDIC Dispatch dispatched appropriate emergency medical service resources, provided appropriate contamination control for the victim and the MEDIC Team, and notified the Carolinas Medical Center - University (CMC-U) on the status of the contaminated injured patient, and then transported the patient to the hospital.
