Preferably, the application should be in the format of Regulatory Guide 7.9. The consolidated application need not include your QA program but special attention should be given to sections on operating procedures, acceptance tests, and maintenance program to assure these sections are current. Note that the certificate of compliance may be conditioned to specifically cite the operating procedures, acceptance tests,-and maintenance program. To ensure continued use of these packages, the consolidated applications (4 copies each) should be submitted at least 30. days prior to the expiration date of the certificate.

Unless an exemption is provided by 10 CFR Part'170.11, an application fee of

$150.00, as required by 10' CFR Part 170.31, should.be included with the request for renewal. This notice of expiration should not be construed that such notice will be provided in the future. Also, please let us know if you do not intend to request renewal of a certificate.
