ISSUANCE OF FINAL SIGNIFICANCE DETERMINATION This is to inform the Commission that a White Significance Determination Process (SDP) finding will be issued on or about February 19, 2009, to Duke Energy Carolinas, LLC (DEC) as a result of inspections at the Oconee Nuclear Station. This White finding, an issue with low to moderate safety significance, involved a maintenance procedure that failed to identify and electrically isolate all main generator automatic voltage regulator (AVR) trip outputs to the main generator lockout relay. Consequently, during AVR maintenance on DEC Oconee Unit 1 generator on April 15, 2008, a main generator lockout occurred, resulting in a loss of power event that ultimately led to a loss of reactor coolant system inventory while in Mode 6 (reactor head closure bolts de-tensioned) on decay heat removal. As such, this finding was considered to have a cross-cutting aspect associated with a procedure, as described in the resources component of the human performance cross-cutting area. Having resulted from an inadequate maintenance procedure for a non-safety-related component (i.e., Main generator AVR) the associated performance deficiency did not involve a violation of regulatory requirements. However, being contrary to a licensee standard, it was characterized as an Initiating Events cornerstone finding.
