Thank you for your letter, dated April 23, 2015 (ML15113B248), in which you responded to NRC Inspection Report 05000285/2015010 and Notice of Violation, dated March 30, 2015 (ML15089A393). The violation involved the failure to:

* provide information to the Commission required by statute or Commission regulation, order or license condition that was complete and accurate in all material respects * conduct a medical examination of an applicant/licensed operator using guidance in American National Standards Institute/American Nuclear Society (ANSI/ANS) Standard 3.4-1996, "Medical Certification and Monitoring of Personnel Requiring Operator Licenses for Nuclear Power Plants* test for the ability to detect products of combustion by provocation testing per ANSI/ANS Standard 3.4-1996, "Medical Certification and Monitoring of Personnel Requiring Operator Licenses for Nuclear Power Plants
