Attachmsnt to U-601004 1 of 1 Clinton Power Station Notification of-Unusual Event On'Tues' day, August 4, 1987, at approximately 1642 hours0.019 days <br />0.456 hours <br />0.00271 weeks <br />6.24781e-4 months <br />, a Notification of Unusual Event (NOUE) was declared in accordance with Emergency. Plan Implementing Procedure EC-02, Emergency Classifications.

This procedure requires that a Notification of Unusual Event be declared for a loss of any engineered safety feature requiring plant shutdown by i

the Technical Specifications.

Clinton Power Station (CPS) was operating at approximately 60 percent power in Mode 1.

The plant was operating in accordance with the

. provisions of ACTION statement b under CPS Technical Specification 3.5 1

.due.to the inoperability of Residual Heat Removal subsystem B for scheduled maintenance.

'At'approximately 1625 hours0.0188 days <br />0.451 hours <br />0.00269 weeks <br />6.183125e-4 months <br />, while. performing CPS No. 9001.01, Control Room Surveillance Log, operators found the Division 1, Low Pressure Core' Spray (LPCS), Reactor Pressure Analog Trip Module (ATM)

(1B21-N697A) reading 0 psig while its sister channel was indicating 999 psig.

In accordance with CPS. Technical Specification Table.3.3.3-1, item A.1.c, ACTION 32, the 1B21-N697A channel,-and associated Low Pressure Core Spray (LPCS), and Low Pressure Coolant Injection Subsystem (LPCI) A were declared inoperable.

With.the above declared inoperable,. combined with the inoperability of RHR B for maintenance, Technical Specification ACTION statement d.3 under CPS Technical Specification 3.5.1 required the plant to be in at least HOT SHUTDOWN within the next 12 hours1.388889e-4 days <br />0.00333 hours <br />1.984127e-5 weeks <br />4.566e-6 months <br />.

At approximately 1642 hours0.019 days <br />0.456 hours <br />0.00271 weeks <br />6.24781e-4 months <br />, the Shif t. Supervisor declared a Notification of Unusual ~ Event based on the loss of any engineered safety feature requiring' plant shutdown.by the Technical Specifications. The State of Illinois was notified of the'NOUE at 1657 hours0.0192 days <br />0.46 hours <br />0.00274 weeks <br />6.304885e-4 months <br />.

At approximately 1700 hours0.0197 days <br />0.472 hours <br />0.00281 weeks <br />6.4685e-4 months <br />, after performing satisfactorily a channel functional test of 1B21-N697A, the device was declared operable.

Low Pressure Core Spray (LPCS) and Low Pressure Coolant Injection Subsystem A (LPCI A) were then declared operable. The plant then exited ACTION statement d.3 under Technical Specification 3.5.1.

At approximately 1704 hours0.0197 days <br />0.473 hours <br />0.00282 weeks <br />6.48372e-4 months <br />, the Shift Supervisor terminated the

-Notification of Unusual Event. At approximately~1715 hours, the State of Illinois was notified that the NOUE was terminated. At approximately 1735 hours0.0201 days <br />0.482 hours <br />0.00287 weeks <br />6.601675e-4 months <br />, the NRC Incident Response Center was notified that an NOUE had been declared and that it was terminated.
