ENS 5100222 April 2015 03:30:0010 CFR 50.72(b)(2)(xi), Notification to Government Agency or News ReleaseOffsite Notification Due to Worker Fatality Not Related to Plant OperationAt approximately 2330 (EDT) on April 21, 2015, a worker collapsed in the turbine building inside the protected area. Initial response by on-site responders found the person unresponsive. Subsequent response by off-site medical responders determined the person had died and the station was notified at approximately 0130 on April 22, 2015. The fatality was due to an apparent personal medical issue and not work related . The individual was not contaminated. The individual was transported off-site via onsite Site Protection personnel. Plant operation was not impacted by the event. The licensee informed Lower Alloways Creek Township and will inform the NRC Resident Inspector.
ENS 508644 March 2015 19:08:0010 CFR 50.72(b)(2)(xi), Notification to Government Agency or News ReleaseOffsite Notification Due to Elevated Tritium Detected OnsiteOn March 3, 2015, PSEG (Public Service Enterprise Group) confirmed the presence of tritium at a concentration of approximately (0.01) microCurie/ml in ice samples taken in a small area immediately adjacent to the north side of the Hope Creek Turbine Building. The New Jersey Department of Environmental Protection Bureau of Nuclear Engineering was notified at 1408 (EST) on March 4, 2015, in accordance with NEI 07-07, Industry Ground Water Protection Initiative. Samples were taken and analyzed in response to ice which was observed to be forming along the exterior of the north Turbine Building wall. Sample results show detectable tritium is confined within the site boundary and there is no impact to the health and safety of the employees or the public. A catch containment was installed in the affected area. PSEG is continuing to evaluate this issue and develop corrective actions. The licensee has notified the NRC Resident Inspector. The local Township will be notified.
ENS 491621 July 2013 12:46:0010 CFR 50.72(b)(2)(xi), Notification to Government Agency or News ReleaseFuel Oil Spill Into a Storm Drain Leading to the Delaware RiverOn July 1, 2013, at 0846 (EDT) the Hope Creek Shift Manager was notified of a fuel oil spill that entered a storm drain leading to the Delaware River. The onsite Fire Department responded to the scene to contain the spill. The spill was terminated and cleanup activities were completed at 0930 (EDT). A 15 minute notification to the State of New Jersey Department of Environmental Protection was required and completed on July 1, 2013, at 0900 (EDT). The unit remains at 100% power. The station has initiated an investigation to determine the cause of the spill and initiate immediate corrective actions. No personnel injuries resulted from the event. This report is being made in accordance with 10 CFR 50.72(b)(2)(xi), notification of other government agencies. The NRC Resident Inspector has been notified. Lower Alloway Creek Township will be notified. The licensee has also notified the National Response Center.
ENS 4755629 December 2011 03:30:0010 CFR 50.72(b)(2)(xi), Notification to Government Agency or News ReleaseOffsite Notification to State of New Jersey Due to a Discharge Into the Delaware RiverAt 2209 EST on 12/28/11 following a planned station service water (SSW) pump swap, a motor operated valve (MOV) failed to close in a water treatment system for the SSW pump that was secured. This was discovered shortly after the pump was removed from service by the field operators. The control room was notified at 2230 EST of the issue and ongoing efforts to troubleshoot and correct the malfunction. The release was terminated by closing a manual isolation valve at 2240 EST. This malfunction resulted in the discharge of approximately 17 gallons of 15% sodium hypochlorite (product) to the idle SSW pump intake which communicates with the Delaware river, The idle SSW pump was restarted at 2240 EST thus withdrawing all residual product from the intake and directing it to plant treatment systems (dechlorination on the cooling tower outfall). A sample was obtained from the Delaware river just outside the intake structure and there was no detectable chlorine in the sample and there were no abnormal conditions noted on the river. Based on Environmental Licensing review, this is reportable to the state of NJDEP (New Jersey Department of Environmental Protection) within 2 hours. The report to the state was initiated at 0017 EST on 12/29/11. The HC Senior NRC Resident has been informed. Affected systems are limited to the Station Service Water (SSW) system and the Sodium Hypochlorite water treatment system. There were no actuations or initiating signals. There was no impact on plant operations and these systems remain fully operable. The failure was limited to the MOV which has been isolated via a manual valve. There was nothing unusual or not understood. All safety related equipment continues to function as required. There were no injuries or reported wildlife impact.Service water
ENS 473235 October 2011 19:53:0010 CFR 50.72(b)(2)(xi), Notification to Government Agency or News ReleaseOffsite Notification Due to Potential Chemical Spill in the Delaware RiverAt 0930 on 10/5/11, a fuse failure in a 1E to non - 1E interface failed causing a false run signal to the water treatment system for a standby station service water pump. The water treatment system responded by opening the injection valve for the out of service pump. With the associated pump out of service, the sodium hypochlorite can communicate with the Delaware river. This was discovered at 1553 and immediately terminated. This resulted in the discharge of approximately 195 gallons of 15% sodium hypochlorite to SSW intake which communicates with the Delaware river. The associated service water pump was started thus withdrawing residual product from the intake. A portion of the discharged sodium hypochlorite may have entered the Delaware river. A sample was obtained from the Delaware river just outside the intake structure and there was no detectable chlorine in the sample and there were no abnormal conditions noted on the river. Based on Environmental Licensing review, this was reportable to the state of NJ within 15 minutes. The report to the state was initiated at 1608 on 10/5/11. Subsequent calls were made to the Hope Creek Senior NRC Resident, the National Response Center and the US Coast Guard. Affected systems are limited to the Station Service Water (SSW) system and the sodium hypochlorite water treatment system. There was no impact on plant operations and these systems remain fully operable. Evaluation is underway to fully understand the impacts of the fuse failure to prevent reoccurrence. There was nothing unusual or not understood. All safety related equipment continues to function as required. There were no injuries or reported wildlife impact.Service water
ENS 469265 June 2011 16:02:0010 CFR 50.72(b)(2)(xi), Notification to Government Agency or News ReleaseOffsite Notification of a Sewage Spill Into a Storm Drain

On June 5, 2011 at 1202 (EDT) the Hope Creek Shift Manager was notified of a sewage spill at the site sewage treatment plain. Approximately 1000 gallons of intimated sewage entered a nearby storm drain. The onsite Fire Department responded to the scene to contain the spill. Offsite support from a local sanitary waste disposal vendor was requested who responded to the site to assist in the cleanup. A 15 minute notification to the State of New Jersey Department of Environmental Protection was required and completed on June 5, 2011 at 1214 (EDT). At 1320 (EDT) the sewage input into the storm drain was terminated. Site personnel continue with the cleanup, with support from the vendor. The spill had no effect on plant operations and the unit remains at 100% power. The station has initiated as investigation to determine the cause of the sewage spill, and initiate immediate corrective actions. No personnel injuries resulted from the event The NRC resident inspector has been notified. Lower Alloways Creek Township has been notified.

Hope Creek has determined that the spill reported to the New Jersey Department of Environmental Protection on 6/5/11 at 1214 (EDT) was not reportable. The spill was considerably smaller than originally believed, and it did not enter the river. Cleanup and pump out of sewage that entered the storm drains is complete. Additionally the sewage that spilled to the ground is also being cleaned up. (The licensee's) site environmental personnel will follow up with NJDEP Southern Enforcement personnel tomorrow morning. To confirm, this spill is not reportable. The licensee states that the spill is now estimated at approximately 25 gallons. The NRC Resident Inspector will be notified. R1DO (Conte) notified.

ENS 4576111 March 2010 22:45:0010 CFR 50.72(b)(2)(xi), Notification to Government Agency or News ReleasePress Release Concerning a Former Plant EmployeeThis notification is being made pursuant to 10CFR50.72(b)(2)(xi) due to the issuance of a press release concerning an individual that previously performed work at Hope Creek Generating Station. 'PSEG Nuclear provided the following statement to Channel 6 News, the ABC-TV affiliate out of Philadelphia, which read as follows: 'Sharif Mobley previously worked as a laborer at PSEG Nuclear for a variety of contractors from 2002 to 2008 mainly during refueling outages for several weeks at a time. This individual satisfied federal security background checks required to work in the US nuclear industry as recently as 2008. While working here, he did routine labor work carrying supplies and assisting maintenance activities. He also worked at other nuclear plants in the region. We are cooperating with law enforcement as part of their investigation as well as the Nuclear Regulatory Commission and other nuclear plant operators.' Sharif Mobley has been the focus of recent news stories due to his activities in the country of Yemen. Hope Creek Generating Station is currently operating at 100%. There is no indication that the individual compromised the security of the station. The licensee will inform the State of Delaware and New Jersey, the Lower Alloways Creek Township and the NRC Resident Inspector.
ENS 4556010 December 2009 18:00:0010 CFR 50.72(b)(2)(xi), Notification to Government Agency or News ReleaseOffsite Notification Due to Onsite FatalityA mechanical maintenance employee died this afternoon from an apparent heart attack. The individual was not actively working at the time and lost consciousness in a shop area. The individual was treated by onsite EMT's who administered CPR and AED (automatic external defibrillator). The death was not the result of any industrial safety issue and did not occur in a contaminated area. He was transported via ambulance to Salem Memorial hospital where he was pronounced dead. The licensee notified the NRC Resident Inspector and local authorities in Lower Alloway Creek Township.
ENS 448946 March 2009 19:40:0010 CFR 50.72(b)(2)(xi), Notification to Government Agency or News ReleaseOffsite Notification Due to Gasoline SpillAt 1440 a gasoline spill occurred while filling a company vehicle from a mobile gasoline truck. The auto-fill valve failed to shut off immediately when the gas tank was full and sprayed / spilled a small quantity (< 1 gallon) of gasoline on the ground. The spill was immediately terminated. This gasoline travelled to the nearest storm drain via water from melting snow. The Operations department was notified at 1445. Initial investigation revealed that the next storm drain in line to the river from the affected storm drain had significant water flow through it from the melting snow and no sheen was observed. There was also no oil sheen visible in the Delaware River. Based on the available indications, a 15 minute report to the State of New Jersey was completed at 1458 in accordance with the Hope Creek Event Classification Guide. The National Response Center was also notified at 1600. Spill recovery actions are in progress and expected to complete before the end of the day. The NRC Resident Inspector has been informed. Plant operation was unaffected, no safety systems were impacted.
ENS 4485012 February 2009 20:00:0010 CFR 50.72(b)(2)(xi), Notification to Government Agency or News ReleaseUs Coast Guard and New Jersey Environmental Hotline Notified Due to Capsized BoatThis notification is based on 10 CFR 50.72(b)(2)(xi) HCGS RAL 11.8.2.a Unusual Conditions Warranting a News Release or Notification of Government Agencies. The US Coast Guard and New Jersey State Environmental Hotline were contacted and provided the following communication. Between 14:00 and 15:00 hours today, an unmanned 25' boat owned by Atlantic Subsea (a PSEG Nuclear contractor) that was tied in the Hope Creek Generating Station barge slip, which is on the Delaware River, at Hope Creek Generating Station overturned and became partially submerged. There were no passengers onboard at the time. Atlantic Subsea reports that the boat contains approximately 40 gallons of gas in a sealed fuel system and 5 gallons of oil in a sealed oil system. Atlantic Subsea does not believe the sealed fuel or oil systems will leak, and there is NO indication of gas or oil leaking into the Delaware River. The gas tank does have a vent, but this vent is fitted with a mechanical vacuum breaker that is not expected to leak. Because of the wind and high seas, PSEG and Atlantic Subsea are unable to safely remove the boat or its contents from the water at this time. It is expected that the boat will be removed from the water tomorrow when the weather improves. PSEG is presently deploying absorbents into the entrance of the barge slip to contain any oil or gas that does leak. In addition, inspections of the water in and around the barge slip will be performed hourly. A notification will be made in the event that a discharge occurs. Atlantic Subsea has informed the Coast Guard. This is a courtesy notification only from PSEG Nuclear LLC's Hope Creek Generating Station. There is no evidence of a leak of petroleum products into the Delaware River. The licensee notified the NRC Resident Inspector.
ENS 4416324 April 2008 03:58:0010 CFR 50.72(b)(2)(xi), Notification to Government Agency or News ReleaseHydrazine Leak Into Storm DrainAt 2340 on 4/23/08 a leak was discovered on a house heating steam boiler that was secured and cooling down. The boiler had been secured at 1930 and there was no leakage noticed at that time. The leak was estimated at 2 gallons per minute. At 2358 it was discovered that this water had reached a storm drain on site. The water contained hydrazine at a concentration of 364 ppb. A 15 minute notification to the state of New Jersey was completed at 0012 in accordance with the plant's Event Classification Guide. Maintenance personnel called to the scene stopped the leak at 0019. Samples are in progress of the sites storm drain outfalls to determine the extent of the discharge. This report is made in accordance with RAL #11.8.2.a. The licensee will notify local township. The licensee notified the NRC Resident Inspector.

ENS 4336817 May 2007 17:40:0010 CFR 50.72(b)(2)(xi), Notification to Government Agency or News ReleaseOffsite Notification Due to Failed Emissions TestI am calling you to provide a notification of a potentially failed air emission test on an auxiliary boiler. There is absolutely no evidence to suggest that any emissions from our boilers caused any harm to human health, welfare, or the environment. The emissions are not continuing and there is no assistance required or requested. The statute requires I provide this notification to assert an affirmative defense or a rescission as appropriate. The New Jersey Department of Environmental Protection Hotline was called to report this, Case number 07-0517-134537, operator 27. During the start-up tuning and testing of our new auxiliary boilers, it appears that a malfunction of equipment during testing resulted in inconclusive test results for NOx loading for our #1 Boiler. For the testing runs, the measured volumetric stack flows were significantly higher than the stoichiometrically calculated stack flows based on fuel flow measurements. While we are still investigating this discrepancy, we preliminarily believe that it was the result of either equipment malfunction, performance of necessary equipment maintenance and equipment start-up and shutdown. The licensee has notified the State (DEP) and will be notifying the NRC Resident Inspector.
ENS 4276411 August 2006 13:55:0010 CFR 50.72(b)(2)(xi), Notification to Government Agency or News ReleaseInadvertent Oca Siren ActivatonAt 0955 EST, the Security Secondary Alarm Station (SAS) operator, in preparation for activity on the site firing range, proceeded to activate the Owner Control Area (OCA) Public Address system to make a site page announcement informing station personnel of range activity. During this process, the SAS operator inadvertently depressed the OCA Siren pushbutton instead of the Public Address pushbutton activating the onsite emergency sirens. The OCA siren system also activates an automatic prerecorded page address which informs non-essential personnel to evacuate the Owner Controlled Area. Approximately 50 to 75 nonessential personnel left the Salem and Hope Creek site, prior to the siren being secured and a page announcement made informing personnel of the inadvertent siren activation. Due to the common OCA, the inadvertent siren actuation affected both Salem and Hope Creek. The siren activation was inadvertent and there was not an actual event requiring site evacuation. Local, County and State agencies were informed of the inadvertent siren actuation. This event is being reported in accordance with 10CFR50.72(b)(2)(xi), Notification of Other Government Agencies. The licensee will notify the NRC Resident Inspector.
