This refers to the special safety inspection conducted by Messrs. P. Hiland,

,

S. Ray, J. Jacobson, H. Walker, and P. Brochman of this office and W. Luckas

of Brookhaven National Laboratory on June 1 through 23, 1989, at Clinton Power

Station authorized by Facility Operating License No. NPF-62 and to the

discussion of our findings with Mr. S. Perry at the conclusion of the

inspection.

The inspection was conducted in response to an event that occurred on June 1,

1989, where, with the plant operating at about 40% power, an ALERT was

declared due to a loss of the "B" reactor recirculating pump seals which

resulted in reactor coolant system leakage in excess of 50 gallons per minute.

In addition to the recirculating pump seal failure, the plant experienced a

-

loss. of the "A" drywell chiller early in the event and a failure of the motor

driven feedwater pump regulating valve during the subsequent plant shutdown.

The enclosed copy of our inspection report identifies the areas examined

during the inspection.

These included a review of operational data during the

event, direct observation of licensee performance during the event and

subsequent recovery actions, direct observation of affected plant equipment,

and your root cause determination.

During this inspection certain of your activities appeared to be in violation

of NRC requirements as described in the enclosed Notice. A written response

is required. While this violation had minimal impact upon the Safety of the

plant our overall evaluation of this violation and NRC observations of the

reactor startup subsequent to the recirculation pump seal failure leads to a

. e are concerned that an apparent problem exists in the

ensuring implementation of commitments. This is particularly disconcerting in

that specific commitments were made to NRC during an onsite meeting of June 16,

1989 yet they were not effectively implemented.
