As a result of the inspection conducted on January 22, 1989 to March 4, 1989 and in accordance with NRC Enforcement Policy (10 CFR Part 2, Appendix C) the J
following violation was identified:
j Technical Specification 6.8 requires that the licensee implement administrative procedures to control modification to safety related systems.
I Work Control Procedure 10.1.9, section 7 3.3 requires that, prior to t
updating, a controlled drawing be marked to reflect an affecting' modification number.
Contrary to the above: On December 27, 1988 controlled drawing FE-1AW, in the control room, was not marked to reflect affecting modification 87-45.
In addition, on March 1, 1989 controlled drawing FM-22A, in the control l
room, was not marked to reflect affecting modification 88-16.
1 Work Control Procedure 10.1.6, section 7.1.7.2 and Operations Department Standing Order, section 7, require that operating procedure (0P) drawings be revised to reflect the as built conditions following a modification.

Contrary to the above: On December 27,1988, OP-468,120V AC Power System, figure 3, had not been revised to reflect the as built condition following modification 87-45.
On December 19, 1988, OP-13, Residual Heat Removal l
System, figure 1, had not been revised to reflect the as built condition following modification 81-36. On November 18, 1988, OP-25, Control Rod Hydraulic System, figure 3, had not been revised to reflect the as built l
condition following modification 85-53.
The above examples collectively constitute a Severity Level IV violation l
(Supplements).
