Gentlemen:
Subject: NRC Inspection of the Trojan Nuclear Plant
This refers to the inspection conducted by Mr. R. C. Barr, Mr. G. Y. Suh and
two reactor inspectors of this office on February 12 through March 25, 1989,
of activities authorized by NRC License No. NPF-1, and to the discussion of
our findings held by Mr. Barr and Mr. Suh with Mr. Yundt and other members of
your staff periodically during the inspection.
Areas examined during this inspection _are described in the enclosed inspection
report. Within these areas, the inspection consisted of selective
examinations of procedures and representative records, interviews with
personnel and observations by the inspectors.
Based on the results of this inspection, it appears that certain of your activities
were not conducted in full compliance with NRC requirements, as set forth in the
Notice of Violation, enclosed herewith as Appendix A.
The concerns related to.these
violations were also a topic of the previous resident inspector report (89-01), and
include concerns on maintenance work control, procedural compliance and adequacy,
and quality organization and management involvement. Management's corrective actions
apparently have not been completely effective, as evidenced by the March 4 steam
generator water level transient.
Your response to this Notice is to be submitted
in accordance with the provisions of 10 CFR 2.201 as stated in Appendix A, Notice
of Violation.
In accordance with 10 CFR 2.790(a), a copy of this letter and the
enclosure will be placed in the NRC Public Document Room.
The apparent violations described in paragraph 6 of the enclosed report will be
subject of separate correspondence.

Two Severity V violations were also identified as discussed in Paragraph 3 and
7 of the enclosed report.
Pursuant to 10 CFR Part 2, Appendix C, a Notice of
Violation was not issued for these two violations. The effectiveness of your
corrective actions associated with these violations will be examined during
future routine inspections.
The responses directed by this letter and the attached Notice are not subject
to the clearance procedures of the Office of Management and Budget as required
by the Paperwork Reduction Act of 1980, PL 96-511.
