Gentlemen:
Subject: Combined Inspection Report Nos. 50-334/89-12 and 50-412/89-13
- This refers to the routine resident safety. inspection by Messrs. J. Beall and
P.' Wilson at the Beaver Valley Power Station in Shippingport, Pennsylvania.
The reporting period covers the time from May 23 - July 7,-1989. The inspec-
. tion' consisted of~ document reviews, interviews and observation of activities.
The findings-were summarized and discussed with you and members of your staff
- at'the conclusion of the inspection.
Based on the_results of this inspection, it' appears that in two instances, ac-
tivities'were 'not conducted in compliance with NRC requirements.
In one in-
stance, . failure to follow procedures accurately led to a reactor trip.
In the
.second. case, corrective actions in response to a previous violation were not
- adequate to control' problems with separations of electrical cable. Note that
ineffective corrective action was the subject of a previous violation in the
inspection report transmitted to you in a December 7, 1988 letter.
Please
respond to these matters in accordance with the enclosed Arpendix A.
