This refers to-the routine safety inspection conducted by Messrs. W. Baunack, .

E. Collins, D. Lew and J. Wechselberger from December 4,1988 through January 14,

1989, at-the Oyster Creek Nuclear Generating Station. .The inspections consisted

of document reviews, interviews and observations of activities as documented in

the enclosed inspection report. 'The' findings.were discussed with you and other

members 'of your staff- at the conclusion of the inspection.

The review conducted of the preliminary! safety' concern process has raised some.

questions regarding the timely and adequate disposition of preliminary' safety con-

cerns.

In addition some concerns have been closed in which valid safety issues

may still exist (see paragraphs 4.0 and 5.0).

Management should consider reviewing

previously? closed safety concerns to determine if other safety issues may still-

be open and valid,

Paragraph 16.0 discusses the source range monitor performance prior to commencing

refueling. The repair activity conducted to ready the monitors for refueling

operations appears to have used shop spare parts.

Inspection Report 50-219/88-33

contained a violation addressing the onsite storage of spare parts.

During the-

inspectors' previous review of spare part storage, your organization assured the

inspector that if any of these parts were to be installed in the-plant, extensive

testing would be conducted to assure that only reliable and properly functioning

parts were used. This event indicates that the quality assurance process to pre-

vent deteriorated or malfunctioning parts from plant installation may not be ade-

quate. Management attention is warranted to ensure that malfunctioning spare parts

are not introduced in the plant and that no shop spare parts which may have suf-

fered some degradation as a result of storage are currently installed in the planL

Your.recent initiatives to complete an error free refueling, divorced from concerns

regarding schedule is commendable. After experiencing a number of defueling errors,

'

the efforts taken by senior site management to effect such a dramatic change during

refueling operations should have a very positive effect on future plant operations.

Within the scope of this inspection, no violations were identified.
